摘要
目的探讨Orem自护模式对顺产后产妇精神、睡眠质量及角色转换的促进作用。方法选择2011年5月～2012年12月于四川省巴中市中心医院行顺产的产妇80例,随机分为研究组和对照组,各40例,对照组给予产后常规护理,研究组采用Orem自护模式对产妇进行护理指导,使产妇了解并掌握自护程序。产后1个月采用症状自评量表(SCL90)评价产妇精神状况,采用PSQI量表评定产妇睡眠质量,并观察产妇角色转换情况。结果护理后研究组SCL90强迫、焦虑、抑郁、敌对、恐惧、偏执等评分均明显低于对照组,差异有统计学意义(P<0.05)。研究组PSQI量表睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、催眠药物、日间功能障碍评分均显著低于对照组(P<0.05),说明睡眠质量提高。两组角色转换优良率比较,研究组高于对照组,差异有统计学意义(P<0.05)。结论 Orem自护模式应用于顺产后产妇可有效改善产后负性情绪,提高睡眠质量,利于产妇心理、生理功能的恢复及产后角色的转变,值得应用推广。
Objective To investigate promoting role of Orem self-care model on mental,sleep quality and role reversal for postpartum maternal.Methods 80 cases of postpartum maternal from May 2011 to December 2012 in Central Hos pital of Bazhong City in Sichuan Province were selected as research objects and were randomly divided into study group and control group,with 40 cases in each group;postpartum maternal in control group was given routine care,study group was given Orem self-care mode on maternal care guide,and made the maternal understand and master self-care program.After one month nursing,self-reporting inventory（SCL90） was used to evaluate maternal mental health,PSQI sleep quality rating scale was used to evaluate sleep quality,and maternal role reversal situation of two groups were observed.Results After strong nursing,the SCL90 obsession,anxiety,depression,hostility,fear,paranoia and other scores of study group were significantly lower than control group,the differences were statistically significant（P 0.05）;PSQI sleep quality,sleep time,sleep time,sleep efficiency,sleep disorders,sleep medication,daytime dysfunction scores of study group were significantly lower than the control group（P 0.05）,it described to improvement of the quality of sleep.Excellent rate of role conversion in study group was higher,the difference was statistical ly significant（P 0.05）.Conclusion Orem self-care model can be applied to improve along postpartum postpartum ma ternal negative emotions,improve sleep quality,favorable maternal psychological,physiological function recovery and postpartum role changes,it is worth of application promotion.
