肝硬化患者血清、腹水中一氧化氮检测及其意义

摘要 目的 :探讨检测肝硬化患者血清、腹水中一氧化氮 (NO)浓度在肝硬化诊断及治疗中的意义。方法 :用硝酸盐还原酶法测定了肝硬化患者血清 (n =4 0 )和腹水 (n =15)、急性肝炎患者血清 (n =50 )、正常对照组 (n =50 )血清中NO浓度 ,结果 :发现肝硬化患者血清NO浓度较正常对照组及急性肝炎组显著升高 (P <0 .0 1) ,肝硬化患者中有腹水者血清中NO浓度较无腹水者升高更为显著 (P <0 .0 1) ,血清中NO的浓度随Child_Pugh分级增高而升高 (P <0 .0 5) ,肝硬化患者血清中的NO浓度与其腹水中NO浓度呈正相关 (r =0 .6 4 7,P <0 .0 5) ,而急性肝炎患者血清中NO浓度虽有升高 ,但与正常对照组比较无显著性差异 (P >0 .0 5) ,结论 :肝硬化时 ,NO合成增加 ,肝功能的损害和腹水的产生与NO的浓度密切相关 ,检测肝硬化患者血清及腹水中NO浓度 ,对临床判断肝硬化病情严重程度和观察疗效具有一定的帮助。 To elucidate the significance of the determination of serum and ascites nitric oxide (NO) for the diagnosis and treatment of patients with hepatic cirrhosis. Methods:By using the method of nitrate reductase,NO in 40 specimens of serum and 15 of ascites in patients with cirrhosis,that in 50 specimens of serum from patients with acute virus_hepatitis and 50 from normal controls,were determined. Results:Serum NO concentration in the patients with cirrohsis was higher than that in normal control and patienrs with acute virus_hepatitis ( P <0 05),and that in cirrhosis patients with ascites was significantly higher than that without ascites ( P <0 01). It was also found that serum NO concentration increased along with the elevation of the Child_Pugh grading in patients with cirrhosis,and the NO concentration in serum was positively correlated to that in ascites ( r =0.674, P <0.05). The serum NO concentration in patients with acute_hepatitis patients was not statistically different from that in normal control group (P>0 05). Conclusion:There is an increased synthesis of NO in cirrhosis,which seems to bear some relationship with the damage of hepatic functions and the production of ascites. Thus,the determination of serum and ascites NO concentration should be helpful for the monitoring of patient′s condition and therapeutic effects.
