摘要
目的探讨血管性痴呆(VaD)患者血清丁酰胆碱酯酶(BuChE)活性与认知功能、执行功能及脑白质病变的关系。方法纳入2009年1月—2012年1月于我院神经内科住院及门诊就诊的25例VaD患者(VaD组)及我院同期体检的健康老年人20例(对照组)。对所有受检者检测血BuChE含量,应用简易精神状态检查量表(MMSE)、画钟试验(CDT)、积木试验(BDT)检测认知及执行功能,采用年龄相关白质改变的评分方法(ARWMCrs)评价脑白质病变程度,并分析相关性。结果 VaD患者血BuChE含量高于对照组〔(7 429±853)U/L比(5 283±965)U/L〕,差异有统计学意义(P<0.05);MMSE〔(16.46±3.52)分比(26.35±3.74)分〕、CDT〔(1.53±0.84)分比(3.65±0.52)分〕、BDT〔(14.88±7.29)分比(38.47±7.31)分〕评分均低于对照组,差异有统计学意义(P<0.01),而全脑〔(11.38±3.63)分比(4.54±2.21)分〕及额区〔(2.59±0.68)分比(1.07±0.53)分〕ARWMCrs评分高于对照组,差异亦有统计学意义(P<0.05)。VaD患者血BuChE含量与MMSE及CDT评分呈负相关(r值分别为-0.415、-0.463,P<0.05),与额叶ARWMCrs呈正相关(r=0.428,P<0.05)。结论VaD患者血BuChE含量增高,与认知功能、执行功能及脑白质病变相关。
Objective To explore the correlation of activity of Butyrylcholinesterase with cognitive function, executive function, and white matter lesions in patients with vascular dementia. Methods Totally 25 patients with vascular dementia and 20 healthy elderlies from physical examination were recruited. Their serum Butyrylcholinesterase contents were measured, and their cognitive functions and executive functions were evaluated with Mini - Mental Status Examination ( MMSE), Clock Drawing Task (CDT) and Block Design Test (BDT). Their white matter lesions were assessed with cranial MRI scanning by using Age - related White Matter Change Rating Scale (ARWMCrs) . Results The content of serum Butyrylcholinesterase in patients with vascular dementia was significantly hither than that of the controls [ (7 429 ± 853 ) U/L vs. (5 283 ±965 ) U/L ] ( P 〈 0.05). The scores of MMSE [ (16.46±3.52) vs. (26.35±3.74)7, CDT [ (1.53±0.84) vs. (3.65±0.52)], BDT [ ( 14. 88 ± 7.29 ) vs. ( 38.47 ± 7. 31 ) ] were significantly lower ( P 〈 0.01 ) ; while the ARWMCrs scores of the whole brain [ (11.38 ±3.63) vs. (4. 54 ±2. 21) 3 and the frontal robe [ (2. 59 ±0. 68) vs. (1.07 ±0. 53) 3 were significantly higher (P 〈 0. 05 ). The content of serum Butyrylcholinesterase in patients with vascular dementia was correlated with MMSE and CDT scores negatively ( P 〈 0. 05 ) and with ARWMCrs score of frontal robe positively ( P 〈 0.05 ) . Conclusion The activity of serum Butyryleholinesterase in patients with vaseular dementia increases, and it has a correlation with executive function, eognitive function and white matter lesions.
