摘要
目的探讨两种不同手术入路治疗累及后髁的复杂性胫骨平台骨折的临床疗效。方法2009年10月～2011年10月对23例累及后髁的复杂性胫骨平台骨折行切开复位内固定。其中采用联合入路13例,前正中入路10例,于术后1、2、3、6、9、12月复查,分别进行临床查体、X线评估和康复指导。骨折愈合后对患者膝关节功能以Lysholm评分系统评分。结果全部病例获完整随访,平均随访15.6月,骨折全部愈合。正中入路组2例皮肤感染,1例发生创伤性骨关节炎。联合入路组临床愈合时间少于正中入路组(P<0.05),术后Lysholm膝关节功能评分两组无统计学差异(P>0.05)。结论联合手术入路治疗累及后髁的复杂性胫骨平台骨折比前正中入路可以缩短术后骨折愈合时间。
Objective To investigate the treatment effect of complex tibia[ plateau fracture involved with poster condyle through different surgical approaches. Methods 23 cases with complex tibial plateau fractures involved with poster condyle were treated with open reduction and internal fixation through different surgical approaches from October 2009 to October 2011. There were 13 cases of combined approach, and 10 cases of anterior median approach. Interviews were taken in 1st, 2nd, 3rd, 6th, 9th or 12th month after operations, and the physical exami- nations, X - ray evaluations, rehabilitation instructions were performed. The evaluation of keen function was taken with Lysholm score system after fracture healing. Results All the cases were followed - up with mean time 15.6 months （12 months - 3 years）, no case of ununion. 2 cases of infection and 1 case of traumatic osteoarthritis in group of anterior median. Fracture healing time in group of combined approach was less than in group of anterior median （ P 〈 0.05）. There was no significant difference according to Lysholm score after operation in two groups （ P 〉 0.05）. Conclusion It can shorten the time of fracture healing of combined approach than anterior mndian approach for treating complex tibial plateau fracture involved with poster condyle
