摘要
目的乳腺纤维瘤病人行乳腺肿物切除术围手术期予抗生素预防感染的病人做药物经济学的分析,总结规范化围手术期用药方式。方法选取2010年至2012年本院乳腺外科病房入院患有乳腺纤维瘤的病人共350例,围手术期预防性应用抗生素,根据用药种类不同及用药时间不同将病人分六组。A1为头孢拉啶1d内停药(50例),A2为头孢拉啶2d停药(55例);B1为头孢呋辛酯1d内停药(55例),B2为头孢呋辛酯2d停药(60例);C1为头孢他啶1d内停药(65例),C2为头孢他啶2d停药(65例)。用成本-效果法进行分析。结果 A1组1d内停药的头孢拉啶(即头孢一代)为围手术期预防性抗生素应用的最佳选择。结论乳腺纤维腺瘤围手术期抗生素的预防使用1d内停药效果经济成本低,其中一代头孢为最优方式。
Objective To summarized the perioperative treatment for standardization to avoid abuse of antibiotics by the cost-effectiveness analysis of preventive antimicrobial agents in excision of breast fibroadenoma at the perioperative period. Methods 350 patients with breast fibroadenoma were selected. All cases were used the antibiotics proactively, and randomly divided into 6 groups according the different kinds and different durations. 55 cases of group A1 received the Cefradine and withdrawal within 1 day; 55 cases of group A2 received the Cefradine and withdrawal within 2 days; 55 cases of group B 1 received the Cefuroxime Axetil and withdrawal within 1 day; 60 cases of group B2 received the Cefuroxime Axetil and withdrawal within 2 days; 65 cases of group C1 received the Ceftazidime and withdrawal within 1 day; 65 cases of group C2 received the Ceftazidime and withdrawal within 2 days. The cost-effectiveness analysis was carried out. Results There were not statistical significant differences of effective rate of infection control among 6 groups, but the cost-effectiveness was different. Groups A1 had the best cost-effectiveness. Conclusions The preventive use of first generation cephalosporins and withdrawal within 1 day has the lowest cost-effectiveness in excision of breast fibrosarcoma at the perioperative period.
