摘要
目的探讨腹腔镜联合胆道镜行保胆取石术的手术方法及疗效。方法回顾性分析2009年10月至2013年2月行腹腔镜联合胆道镜保胆取石术168例患者的临床资料。结果 168例患者中顺利完成保胆取石术166例,中转腹腔镜胆囊切除术2例。术后均无胆漏、胆囊及胆道出血、腹腔感染、胆石残留等并发症,患者均痊愈出院。术后随访4-36个月,结石复发2例,复发率为1.2%。结论腹腔镜联合胆道镜行保胆取石术具有创伤小、康复快、并发症少、结石残留率低等优点。严格掌握手术适应证,腹腔镜联合胆道镜保胆取石术是安全、有效的。
Objective To study the operative method and curative effect of laparoscopy combined with choledochoscopy in gallbladder-preserving cholelithotomy.Methods To retrospectively analyze the clinical data in 168 cases of laparoscopy combined with choledochoscopy for gallbladder-preserving cholelithotomy from October 2009 to February 2013.Results Among 168 cases,166 cases were successful in cholecystolithotomy and 2 cases were converted to laparoscopic cholecystectomy.No post operative complications like bile leakage,bile duct injury or bleeding,abdominal infection or gallstone residue appeared and all patients were cured and discharged from hospital.After potoperative 4-36 months follow-up,2 cases had gallstone recurrence with the recurrence rate of 1.2%.Conclusion Combination of laparoscopy and choledochoscopy for cholecystolithotomy has the advantages of less trauma,quick recovery,fewer complications and low gallstone residual rate.Strictly mastering the operation indications,cholecystolithotomy by laparoscopy combined choledochoscopy is safe and effective.
