摘要
目的研究肺外神经内分泌癌(NEC)的影像表现,提高对该病的诊断水平。方法回顾性分析12例经病理确诊肺外神经内分泌癌患者的临床及影像资料。结果12例病例,病变分别位于胃、纵隔、胰腺、扁桃体、喉部、肝脏、壶腹部、十二指肠、鼻腔、鼻咽及腹膜后等,呈圆形或不规则形,12例均为单发;10例平扫密度及信号不均匀,中心见不同程度坏死区;增强扫描动脉期肿瘤实性部分明显强化,门脉期强化程度改变不明显,坏死区未见强化;余病灶平扫密度及信号均匀,增强扫描均匀性明显强化,未见坏死;5例肿瘤有周围组织浸润及远处淋巴结转移征象。结论肺外神经内分泌癌的影像特征主要为病灶呈圆形或不规则形,边界较清晰,平扫密度或信号不均匀,可见坏死,强化明显,结合临床信息行多层螺旋CT及MRI扫描对于肺外NEC的术前诊断具有一定价值。
Objective To investigate the imaging findings and diagnostic values of etra-pulmonary neuroendocrine carcinoma.Methods Imaging and clinical data of 12 patients with extra-pulmonary neuroendocrine carcinoma pathological confirmed were retrospectively analyzed.Results Among the 12 cases,the lesions were located in the stomach,mediastinum,pancreas,tonsil,larynx,liver,duodenum,ampulla,nasal cavity,nasopharynx and the peritoneal cavity.The lesions were in round or unregular shape and all of which were singly located.Unhomogeneous density or signal in plain scan imaging were noticed in 10 cases with varying degrees of necrosis in the center regions.The solid part of tumors were obviously intensified in the arterial and venous phase,no enhance were observed in the necrosis region.Surrounding tissue involvement and distant lymph nodes metastasis were observed in 5 cases.Conclnsion The main characteristics of the extra-pulmonary neuroendocrine carcinoma showed a round or unregular shape with a clear margin.The lesions included unhomogeneous density or signal in plain scanning and obvious necrosis in the center.Dynamic enhanced multi-slice spiral CT or MRI scanning combined with clinical data would possess high diagnostic value for siagnosis of etra-pulmonary neuroendocrine carcinoma.
