经皮球囊扩充椎体成型术治疗无神经损伤胸腰椎骨折的临床观察 被引量:8

摘要 目的探讨经皮球囊扩充椎体成型术对无神经损伤胸腰椎骨折的治疗效果。方法选择我院2009年1月~2012年1月间收治的32例无神经损伤的稳定性胸腰椎压缩性骨折患者,采用经皮球囊扩充椎体成型术治疗,术后随诊1年,比较手术前后脊柱后凸Cobb角、椎体前缘压缩比例、疼痛VAS评分及工作状态Denis评分的情况。结果术前32例患者的Cobb角为18.2°±3.1°,椎体前缘压缩比例为68.5%±7.1%,术后各随访时间点的Cobb角和椎体前缘压缩比例与术前比较,差异有统计学意义(P<0.05)。术前32例患者的疼痛VAS评分为8.31±1.23,工作状态Denis评分为4.02±0.71,术后各随诊时间点的VAS评分和Denis评分与术前比较,差异有统计学意义(P<0.05)。结论对于无神经损伤的稳定性胸腰椎压缩性骨折来说,经皮球囊扩充椎体成型术能够明显减少患者的卧床时间、减轻患者的疼痛程度、较快恢复工作状态,是一种较理想的治疗方式。 Objective To discuss the effect of percutaneous kyphoplasty in the treatment of thoracolumbar vertebra frac- ture without neurological deficit. Methods 32 cases of thoracolumbar vertebra fracture patients without neurological deficit were treated by pereutaneous kyphoplasty. Bisegmental Cobb angle, anterior vertebral body height ratio, level of pain and physi- cal ability were analyzed before and after the operation. Results The Cobb angle of patients before operation was 18.2°±3.1°, the anterior vertebral body height ratio was 68.5%±7.1%. There was difference between before and after the operation (P〈0.05). The VAS scores of level of pain and Denis scores of physical ability were 8.31±1.23 and 4.02±0.71. There were differences be- tween before and after the operation by VAS scores and Denis scores (P〈0.05). Conclusion For the thoracolumbar vertebra fracture patients without neurological deficit, percutaneous kyphoplasty is ideal.
