青年士兵腰椎间盘盘源性下腰痛的CT椎间盘造影研究 被引量:1

摘要 目的研究CT腰椎间盘造影术(CTD)对青年士兵盘源性下腰痛的诊断价值。方法对28例盘源性下腰痛的青年士兵患者行CT腰椎间盘造影术,共计造影79个腰椎间盘,分析其CTD分型、对比剂注射剂量、诱发一致性疼痛的关系。结果 CTD显示盘源性下腰痛的青年士兵患者其椎间盘内破裂主要为Ⅱ型和Ⅳ型,其分型、对比剂注射剂量与诱发一致性疼痛具有相关性,阳性和阴性椎间盘对比剂平均注射剂量具有明显差异(t=7.50,P<0.01)。结论 CTD能直接显示腰椎间盘纤维环破裂的形态,确定责任椎间盘,能够为盘源性下腰痛的治疗提供有效的依据。 Objective To study the diagnostic value of discogenic low back pain (DLBP) of young soldiers with CT discography (CTD). Methods CTD was successfully applied to 79 discs in 28 young soldiers with DLBP. We evaluated the correlations among CTD group, dose of contrast medium and concordant pain. Results Internal disc disruption (IDD) mainly included CTD group II and IV in the young soldiers with DLBP. There were correlations among CTD group, dose of contrast medium and concordant pain. The difference in the dose of contrast medium was significant between positive and negative discs ( t = 7.50 ,P 〈 0.01 ). Conclusion CTD can show the shape of internal disc disruption directly, identify the symptomatic disc, and provide more information for treat- ment of DLBP.
