摘要
医改的3年推进,我国城市社区卫生服务有了大发展,进入了巩固提高期。通过调查,结果显示居民对国家卫生服务政策、社区卫生服务工作较为满意。伴随着医疗体制改革的深化,一些深层次的矛盾突显。西方发达国家经过了数百多年的发展,社会保障制度较为完善,特别是社区卫生服务方面有许多成功的经验可供汲取。本文通过分析四种医疗保障模式的代表国英国、美国、德国和新加坡在医疗保障模式和社区卫生服务管理模式,总结了我国医疗保障和社区卫生服务工作的现状,借鉴国外经验,提出了我国社区卫生服务管理模式应符合我国的基本国情,要加强"首诊制",落实"守门人"制度;加强队伍建设,提高社区卫生服务机构服务能力;完善经费投入机制,保证公益性;进一步健全完善医疗保险体制机制和社会救助机制,提高保障能力。
After three years of health care reform, our urban community health service obtains large development and goes into a consolidating and improving period. Some surveys showed that the residents satisfied with the national health policy and commu- nity health services. But along with the medical system reform, some deep-seated contradictions were highlighted. The social secur- ity system of western developed countries is relatively complete after more than 100 years of development, especially there are a lot of successful experiences of community health services we can learn from them. The article analysis international models of medi- cal insurance and the community health service management, which sourced from four countries（ Britain, USA, Germany and Sin- gapore）. Meanwhile,it Summarizes the current situations of our country, and maintains our community health service management model should accord with the fundamental national condition of our country, strengthen ＂the first consultation system＂, implement ＂ gatekeeper＂ system ; strengthen team construction, improve the community health service ; improve the funding system, ensure the public welfare, further perfect the medical insurance system and social assistance system, and improve the capability of safeguard.
