摘要
目的了解分析中风病程中发热的原因及特点,通过护理对策,避免或减少发热现象的发生。方法对78例中风后发热病人常见的临床原因分为四种,即吸收热、中枢热、脱水热、感染热,然后给予早期护理干预如观察,检查,把关,同时采取护理干预方法包括对症处理、降温方法、降温过程中的护理。结果计有吸收热5例,中枢热12例,脱水热3例,感染热54例,不明原因4例。本组发热病例仅占25%,明显低于2010年全国神经系统疾病临床诊疗新进展研讨会报告的60%～80%。结论吸收热可自行缓解,多数为脑出血病人。中枢热为出血后脑水肿,使丘脑下部体温调节中枢受到损害而引起的发热。脱水热为应用大量脱水剂利尿导致脑组织严重脱水、脑细胞和体温调节中枢受损引起的发热。感染热最为常见,本组54例通过早期护理干预明显减低了发热的比例。
Objective To explore the causes and features of the fever post stroke and during hospitalization;to prevent or lessen the occurrence of fever by nursing strategy.Methods The 78 patients with fever were divided into four kinds,namely attracts,the main center heat,the dehydrated heat,the infection heat endothermically according to their own characterisics,they were given proper early nursing interventions including： ① closes observation;② physical examination;③ strictly keep the key points.Results The idea had attracted the endothermic 5 examples,the main center hot 12 examples,dehydrates the hot 3 examples,infects the hot 54 examples,unclear reason 4 examples.The rate of occurrence of fever post stroke was greatly reduced,only 25% and much lower than that reported（60%-80%） in The Second National Meeting on Therapeutic Progress of Neurologic Disease.Conclusion Attracts endothermically may alleviate voluntarily,most for cerebral hemorrhage patient.After the main center heat and the hemorrhage the hydro-cephalus,caused hypothalamus the thermotaxic center to receive giving off heat which the harm causes.Dehydrates the heat and the application massive dehydrator diuresis caused the brain organization serious dehydration,the brain cell and the thermotaxic center damage caused giving off heat to be most common this group of 54 examples obviously to decrease the proportion through the early nursing intervention which gave off heat.
