摘要
目的评价窝洞的充填因素对牙折的影响。方法应用流行病学调查方法,收集2009年6月-2011年7月来煤炭总医院就诊已进行窝洞充填超过1年的1256颗后牙,其中发生238颗牙折的临床病例,分析影响其牙折发生的相关因素。结果银汞充填的牙齿783颗,有182颗牙齿发生折裂(23.2%)。金嵌体的牙齿95颗,有23颗(24.2%)发生折裂,而树脂充填牙数为312颗,折裂牙数为26颗(8.3%),瓷嵌体牙齿66颗,发现折裂的牙数为7颗(10.6%)。银汞合金及金嵌体充填较树脂与瓷嵌体充填牙折裂的发病率要高,差异有统计学意义(P<0.05)。就充填的类型来看,Ⅱ类充填的牙齿576颗,发现127颗牙齿(22.0%)折裂,Ⅰ类充填的牙齿680颗,其中111颗折裂牙(16.3%),Ⅱ类充填较Ⅰ类充填折裂的发生率高,差异具有统计学意义(P<0.05)。结论充填材料的选择和充填的类型是影响牙折的重要因素。
Objective To explore the influence factor of cavity filling on posterior fractured teeth.Methods Through epidemiological investigation,1256 posterior teeth were collected,cavity filling was diagnosed beyond one year and factors related to fractured teeth was analyzed.Results Compared with resin（26 in 312,8.3%） or porcelain（7 in 66,10.6%）,the use of materials such as gold（23 in 95,24.2%） or amalgam（182 in 783,23.2%） increased the occurrence of longitudinal tooth fractures,the difference between them was significant（P〈0.05）.As the class Ⅰ or Ⅱ restorations,the class Ⅱ restorations（127 in 576,22.0%） were associated with more cracks than class Ⅰ restorations（111 in 680,16.3%）,the difference between the two groups was significant（P〈0.05）.Conclusion Cavity filling materials,type of cavity were important causes of fracture tooth.
