摘要
目的系统评价百令胶囊联合常规治疗对早期糖尿病肾病的疗效及安全性。方法计算机检索Cochrane图书馆(2012年第1期)、EMBase、PubMed、中国期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)、中文科技期刊全文数据库(VIP)关于百令胶囊联合常规治疗对早期糖尿病肾病的随机对照试验(RCT)(各数据库检索时间均从创建至2012年6月,用RevMan5.14软件对数据进行Meta分析。结果共纳入29篇RCT,包括1 601例患者。与对照组相比,治疗组尿蛋白排泄率较低[MD=-27.41,95%CI(-32.82,-21.99),P<0.000 01],24h尿蛋白值较低[MD=-48.95,95%CI(-67.66,-30.25),P<0.00001],血肌酐值较低[MD=-6.34,95%CI(-10.94,-1.75),P<0.000 01],尿素氮值较低[MD=-0.49,95%CI(-0.97,-0.01),P=0.04],有效率较高[OR=4.98,95%CI(3.12,7.95),P<0.000 01];空腹血糖值略低,但差异无统计学意义[MD=-0.02,95%CI(-0.28,0.23),P=0.86]。两组均未发现严重不良反应。结论基于现有临床证据,百令胶囊联合常规治疗对早期糖尿病肾病有效、安全性好。但由于纳入研究质量不统一,此结论尚需更多大样本、高质量临床随机对照试验予以证实。
Objective To evaluate the effect and safety of Bailing Capsule combined with the conventional treatment for treating early disbetic nephropathy.Methods The Cochrane Library（ Issue 1,2012）,EMbase,PubMed,CBM,CNKI and VIP（ from their establishment to October 2012） were retrieved by computer for collecting the randomized controlled trials（ RCTs） of Bailing Capsule combined with the conventional treatment for treating early disbetic nephropathy.The data were performed the meta analysis by RevMan 5.14 software.Results Twenty-nine RCTs involving 1 601 patients were ultimately identified.The Results of meta-analyses showed that,compared with the control group,the urinary albumin excretion rate（ UAER） of the Bailing Capsule group was lower than that of the control group [ MD =-27.41,95% CI（-32.82,-21.99）, P 0.000 01 ];the 24h UMA of the Bailing Capsule group was lower than that of the control group,[ MD =-48.95,95% CI（-67.66,-30.25）, P 0.000 01 ];the serum SCr of the Bailing Capsule group was lower than that of the control group,[ MD =-6.34,95% CI（-10.94,-1.75）, P 0.000 01 ];the BUN of the Bailing Capsule group was lower than that of the control group,[ MD =-0.49,95% CI（-0.97,-0.01）, P = 0.04 ];the effective rate of the Bailing Capsule group was higher than that of the control group,[ OR = 4.98,95% CI（3.12,7.95）, P 0.000 01 ].The fasting plasma glucose（ FPG） of the Bailing Capsule group was lower than that of the control group without statistically significant difference between them [ MD =-0.02, 95% CI（-0.28,0.23）, P = 0.86 ].No serious adverse reactions were found in the two groups.Conclusion Based on the present clinical evidences,Bailing Capsule combined with the conventional treatment for treating early disbetic nephropathy is effective with good safety.Howerver,due to the quality disunity of the included studies,this Conclusion need more large sample size,high-quality RCTs to confirm.
