摘要
目的:探讨电子支气管镜检查(肺泡灌洗术)对呼吸衰竭患儿在撤除有创呼吸机过程中的影响。方法:收集本院2009年6月至2012年6月期间因呼吸衰竭行有创机械通气患儿病例共116例,其中行支气管镜肺泡灌洗术76例(干预组),无行支气管镜肺泡灌洗术共40例(对照组),对比有创机械通气时间,对比吸入氧浓度(FiO2)、呼吸频率(RR)、吸气峰压(PIP)、呼气末正压(PEEP)等呼吸机参数下调的时间,同时对比两组患儿血气分析及胸片改善情况。结果:干预组患儿有创机械通气时间、下调FiO2时间、下调PIP时间均较对照组患儿明显缩短,差异具有统计学意义(P<0.05),但下调RR时间与下调PEEP时间较对照组间差异无统计学意义(P>0.05);干预组患儿血气及胸片改善情况明显快于对照组患儿,差异均具有统计学意义(P<0.05)。结论:电子支气管镜肺泡灌洗术可明显加快降低呼吸衰竭患儿有创呼吸机参数时间,改善患儿血气及胸片情况,缩短有创呼吸机使用时间。
Objective To investigate the influence of electronic bronchoalveolar lavage(BAL)on children with respiratory failure in withdrawing invasive ventilator.Methods The data of 116 cases receiving invasive mechanical ventilation were collected from 2009.6 to 2012.6 in Guangdong Women and Children Hospital,BAL was performed in 76 cases as intervention group,other 40 cases receiving suction method only as control group.The time of using ventilation,the time of reducing inspired oxygen concentration(FiO 2),respiratory rate(RR),peakinspiratory pressure(PIP) and end-expiratory pressure(PEEP) were compared,while blood gas and chest X-rays improvement were compared in two groups.Results The time of ventilation,reducing FiO 2 and reducing PIP in intervention group was significantly shorter than the control group(P〈0.05),but the time of reducing RR and PEEP was not significantly shorter than the control group(P〈0.05).The improvement of blood gas and chest X-rays of intervention group was significantly faster than the control group(P〈0.05).Conclusion BAL can significantly speed up the time of reducing invasive ventilation parameters on children with respiratory failure,improve the blood gas and chest X-ray situation and then shorten the time to of using ventilator.
