微型岛状皮瓣修复手部软组织缺损的临床应用 被引量:6

摘要 目的报道微型岛状皮瓣修复手部软组织缺损在临床上的应用效果。方法选取2005年3月至2008年3月东莞市塘厦医院收治的手部软组织损伤患者164例,将未引进微型岛状皮瓣修复手部软组织缺损技术前的86例患者(皮瓣来源胸腹部或前臂)作为传统组,利用微型岛状皮瓣修复手部软组织缺损的78例患者为微型组。观察比较两种移植方法的疗效及远期恢复情况。结果微型组78例患者中,76例皮瓣良好成活,皮瓣质地、弹性及外观均达到满意效果,关节功能恢复良好,另外2例皮瓣出现远端局部坏死,经换药痊愈;而传统组86例患者中,20块皮瓣做了二期皮瓣修整术(削薄皮瓣)。微型组临床疗效优于传统组(Z=2.56,P<0.05)。结论选择微型岛状皮瓣修复不同部位的手部损伤,皮瓣获取较容易,损伤较小,术后手的功能及外形恢复良好。 Objective To report the clinical application effect of miniature island flap in repairing hand soft tissue defects.Methods From Mar.2005 to Mar.2008,86 patients(flap source of the chest and abdomen or forearm) before the introduction of micro-island flap repair of hand soft tissue defect technology by Dongguan City Tangxia Hospital were chosen as the traditional group,and 78 cases who used miniature-island flap repair of hand soft tissue defect were chosen as a mini-group.Eefficacy and long-term recovery of the two transplant methods were observed and compared.Results Among the 78 patients of the mini-group,76 flaps had good survival,flap texture,elasticity and appearance all achieved satisfactory results,a good recovery of joint function was seen,distal partial necrosis occurred in the other two cases which were healed after dressing changes;among the 86 patients of the traditional group,20 flaps needed a second-time flap trimming technique(thinning flap).The clinical efficacy in mini-group was superior to the traditional group(Z = 2.56,P 0.05).Conclusion Miniature island flap repairing the different parts of the hand injury has the advantages of easy access of flap,less damage and good function and appearance recovery.
