摘要
目的探讨甲状腺手术中甲状旁腺保护及术后低钙血症的处理方法。方法 70例患者其中双侧腺叶全切除术5例,一侧腺叶全切除术+对侧次全切除术20例,双侧次全切除术13例,一侧腺叶全切除+对侧部分切除术12例,单侧腺叶次全切除术20例。术中注意甲状旁腺的保护。结果在70例甲状腺手术中,23例发生低钙血症,发生率32.86%。其中双侧腺叶全切除术5例,一侧腺叶全切除术+对侧次全切除术14例,双侧次全切除术2例,一侧腺叶全切除+对侧部分切除术1例,单侧腺叶次全切除术1例。给予对症治疗后,均得到有效控制。结论甲状腺手术后易发生低钙血症,患者多为暂时性低血钙,部分患者有低钙症状,适当补钙,均可得到较好的预后。
Objective It is thyroid surgery, parathyroid protection and treatment of postoperative hypocalcemia. Methods 70patients, including bilateral lobe resection 5 eases, the side lobe subtotal hysterectomy ＋ contralateral subtotal resection in20 cases, 13 cases of bilateral subtotal resection, the side lobe subtotal ＋ contralateralpartial resection of 12 cases the unilateral lobe subtotal resection of 20 cases. Pay attention to the protection of parathyroid surgery. Results 70cases of thyroid surgery, 23 cases of hypocalcemia, the incidence of 32.86%. Bilateral lobe resection 5cases, the side lobe resection ＋ eontralateral subtotal resection of 14 cases, bilateral subtotal resection of two cases, one side lobe subtotal ＋ contralateral partial nephrectomy cases of unilateral lobe subtotal resection. Given symptomatic treatment, have been effectively controlled. Conclusion Prone to hypocalcemia after thyroid surgery in patients with transient hypocalcernia, low calcium symptoms in some patients, appropriate supplements, may get a better prognosis.
