摘要
目的了解老年心力衰竭患者的窦性心律震荡(HRT)变化情况,探讨其对心力衰竭患者的诊治及预测预后的临床意义。方法选择某三甲医院住院和门诊的慢性心衰患者100例及同期在该院体检的健康人62例为研究对象。对所有研究对象经超声心动图测量左室射血分数(LVEF),同时进行动态心电图检查,计算心率变异性的时域法指标SDNN、平均心率以及震荡初始(TO)和震荡斜率(TS)。结果心衰患者组TO高于对照组,而TS值、LVEF和HRV均低于对照组(P<0.05)。重度心衰组TS、HRV、LVEF值均小于轻度心衰组(P<0.05)。慢性心衰组TO与HRV呈负相关(P<0.05),TS与HRV、LVEF呈正相关(P<0.01)。结论 HRT可作为慢性心衰患者的重要临床检测指标,也可作为预测CHF患者预后的指标之一。
Objective To understand the status of HRT and explore its clinical value of diagnosis, treatment and prognosis in elderly patients with chronic heart failure. Methods All 100 patients with CHF and 62 healthy controls were chosen from a third - grade class - A hospital. The left ventrieular ejection fractions （LVEF） of all objects were measured by echocardiography. Meanwhile, the ambulatory elec- trocardiograms were taken to calculate their SDNN, average heart rate, turbulence onset （TO） and turbulence slope （TS）. Results Com- pared with the controls, the patients with chronic heart failure had higher TO, but lower TS, LVEF and HRV （ P 〈 0.05 ）. TS, LVEF and HRV were lower in the serious patients than those in mild patients with CHF（P 〈0.05）. TO had negative correlations with HRV, while TS had positive correlations with HRV and LVEF of the patients. Conclusion HRT can be not only an important index of clinical examination, but also a predictor of prognosis in elderly patients with CHF.
