集中配液时胶塞微粒污染的调查分析 被引量:8

Effect of formation on debris of rubber stopper for injection bottle in pharmacy intravenous admixture services

摘要 目的观察不同穿刺方法对静脉药物配置时胶塞脱屑的影响。方法以16号单侧孔圆锥形针头与16号双侧孔圆锥形针头为观察对象,分别观察45、60、90、120度进针取药时橡胶塞碎屑形成情况。结果使用16号单侧孔及双侧孔针头用4种角度穿刺密闭瓶胶塞取药,16号单侧孔针45、60、90、120度进针的脱屑率分别为23.0%、20.3%、9.9%、28.6%;16号双侧孔针45、60、90、120度进针的脱屑率分别为26.1%、24.3%、17.6%、27.5%;16号单侧孔针90度方向穿刺胶塞碎屑形成率最低为9.9%,差异有统计学意义;不同的穿刺角度对于单侧孔及双侧孔针头穿刺的脱屑发生率均有显著影响,其中90度进针时的发生率较低,分别为9.9%和17.6%,可明显减少输液胶塞微粒的污染。结论 16号单侧孔针穿刺脱屑率低于16号双侧孔的脱屑率,16号单侧孔针头以90度从正面垂直穿刺胶塞瓶内取药,可明显减少胶塞脱屑数量,降低医院输液反应的发生,值得推广应用。 OBJECTIVE To observe the effects of different puncture methods on the formation of debris of rubber stopper for injection bottle in pharmacy intravenous admixture services(PIVAS).METHODS The rubber stopper debris formation for injection bottle was compared with the No.16unilateral hole pinhead to puncture with the No.16bilateral holes pinhead at different angle(45degree,60degree,90degree and 120degree)puncturing.RESULTS The rates of rubber stopper debris were 23.0%,20.3%,9.9% and 28.6% for the No.16unilateral hole pinhead and 26.1%,24.3%,17.6% and 27.5%for the No.16bilateral hole pinhead,respectively,at 45 degree,60degree,90degree,120degree angle puncturing for aspiration.The No.16unilateral hole pinhead with 90degree angle puncturing produced the least rubber stopper debris with the formation rate of 9.9%,the difference was statistically significant(P〈0.05).The various puncture angle influenced on the rate of rubber stopper debris of either the No.16unilateral hole pinhead or the at 90degree angle puncturing,the incidence rates of the rubber stopper debris were 9.9% and 17.6%,respectively,which significantly reduced the particulate contamination.CONCLUSIONThe ratio of rubber stopper debris with No.16unilateral hole pinhead puncturing is lower than of No.16bilateral hole pinhead.The ways of the No.16unilateral hole pinhead at 90degree angle puncturing(the positive vertical direction)can significantly reduce the rubber stopper debris,and decrease the rate of infusion reaction.The improvement ways of puncturing should be widely applied for injection bottle in PIVAS.
