摘要
目的比较放疗同步XELOX与放疗同步XP方案治疗局部晚期胃癌的近期疗效及不良反应。方法将90例局部晚期胃癌或胃食管交界腺癌患者随机分为奥沙利铂组与顺铂组,两组皆给予常规分割调强放疗,在此基础上,奥沙利铂组给予XELOX方案,顺铂组给予XP方案。放疗结束后4～6w复查CT,比较两组近期疗效、KPS评分、疼痛缓解率及不良反应发生率。结果奥沙利铂组KPS评分改善率明显高于顺铂组,而疼痛缓解率差异无统计学意义;两组不良反应主要包括白细胞减少、贫血、血小板减少、恶心、呕吐、纳差、腹泻、外周神经毒性及手足综合征,且以Ⅰ、Ⅱ度为主。奥沙利铂组Ⅰ度血小板减少及Ⅱ度恶心、呕吐、纳差显著低于顺铂组,而Ⅱ度外周神经毒性显著高于顺铂组。结论放疗同步XELOX与放疗同步XP方案治疗局部晚期胃癌近期疗效相当,但XELOX同步放疗方案能更好地改善生活质量,提高KPS评分,且不良反应轻。
Objective To compare the short-term effects and adverse reactions of XELOX and XP regimens in the treatment of local advanced gastric cancer when combined with concurrent radiotherapy. Methods Ninety patients with local advanced gastric cancer or adenocarcinoma of esophagogastric junction were randomly divided into oxaliplatin group and cisplatine group. Based on routine fractionated intensity-modulated radiotherapy, patients in oxaliplatin group was treated with XELOX regimen,while the ones in cisplatine group were given XP regimen. CT was made again 4-6 weeks after radiotherapy treatment, and short-term effects, KPS score, pain relief rate and incidence of adverse reactions were anayzed in two groups. Results The improvement rate of KPS score in oxaliplatin group was obviously higher than that in cisplatine group, whereas there was no significant difference in terms of pain relief rate. Adverse reactions including decreased white cells, anemia, decreased platelet, nausea, vomiting, poor appetite, diarrhea, peripheral nerve toxicity and hand-foot syndrome, existed mainly at grades I, Ⅱ. The Grade I platelet count decreased and nausea, vomiting and Grade Ⅱ poor appetite in oxaliplatin group were significantly lower than those in cisplatine group,while Grade Ⅱ peripheral nerve toxicity was conspicuously higher than that of cisplatine group. Conclusion The therapeutic effects of XELOX and XP regimens are similar in the treatment of local advanced gastric cancer when combined with concurrent radiotherapy, however, the former can improve the quality of life better,with a higher KPS score and mild adverse reactions.
