摘要
目的探讨临床护理路径在单一前外侧切口治疗Pilon骨折中的临床效果。方法将广东省中山市黄圃人民医院2011年2月～2012年10月收治的40例胫骨Pilon骨折患者按照其护理措施分为实验组与对照组,每组各20例,实验组给予临床护理路径进行护理,对照组给予常规的护理措施。比较两组患者的满意度、住院时间、住院费用、首次锻炼时间、出院时踝关节屈伸活动度以及随访2个月时患者踝关节屈伸活动度。结果对照组患者的总满意度为90%,实验组患者的总满意度为100%,差异有统计学意义(χ2=7.35,P<0.05);对照组患者的住院费用为(5641.4±223.5)元,实验组为(5712.3±199.6)元,差异无统计学意义(t=1.78,P>0.05);对照组患者住院时间为(28.5±1.8)d,实验组为(23.2±1.5)d,差异有统计学意义(t=11.27,P<0.05);实验组患者首次锻炼时间为(0.7±0.3)d,对照组为(2.2±1.1)d,差异有统计学意义(t=4.92,P<0.05);实验组患者出院时踝关节屈伸活动度为(52.5±7.5)°,对照组为(38.1±11.5)°,差异有统计学意义(t=8.81,P<0.05);实验组患者随访2个月时踝关节屈伸活动度为(64.2±8.1)°,对照组为(50.3±8.9)°,差异有统计学意义(t=9.15,P<0.05)。结论临床护理路径在单一前外侧切口治疗Pilon骨折中效果显著,值得推广应用。
Objective To explore the clinical effect of clinical nursing path in a single anterolateral incision in the treatment of Pilon fractures. Methods 20 eases of tibial Pilon fracture patients were divided into the experimental group and control group according to the nursing measures in Huangpu People＇s Hospital of Zhongshan City in Guangdong Province from February 2011 to October 2012, each group of twenty cases, the experimental group was given the clinical nursing path for nursing, the control group was given the conventional nursing measures. The patients＇ satisfaction, length of hospital stay, cost for the first time in hospital, exercise time, discharge ankle symptoms and activity of ankle motion followed up for 2 months were compared between the two groups. Results The total satisfaction was 90% in the control group, the total satisfaction was 100% in the experimental group, the difference was statistically significant （χ^2=7.35, P 〈 0.05）; the hospital cost was （5641.4±223.5） yuan in the control group, （5712.3±199.6） yuan in the experimental group, the difference was statistically significant （t = 1.78,P 〉 0.05）; the hospital stay was （28.5±1.8） d in the control group, and the experimental group was （23.2±1.5） d, the difference was statistically significant （t = 11.27, P 〈 0.05）; the patientls first time for exercise was （0.7±0.3） d in the experimental group, and （2.2±1.1） d in the control group, the difference was statistically significant （t = 4.92, P 〈 0.05）; the patients in the hospital ankle stretch activity was （52.5＋7,5）° in the experimental group, and （38.1±11.5）° in the control group, the difference was statistically significant （t = 8.81, P 〈 0.05）; the follow-up of pa.tients with 2 months ankle stretch activity was （6412±8.1）° in the experimental group, and （50.3±8.9）° in the control group, the difference was statistically significant （t = 9.15, P 〈 0.05）.Conclusion The treatment effect is remarkable of clinical nursing pathway in a single anterolateral incisions in Pilon fracture, it is worth for spreading application.
