摘要
目的观察腹腔镜联合胆道镜治疗胆囊结石合并胆总管结石的临床效果,探讨其临床应用价值。方法选取我院2010年9月至2012年10月收治的胆囊结石合并胆总管结石患者102例,分为对照组和观察组各51例,对照组采用开腹手术,观察组采用腹腔镜联合胆道镜手术治疗,比较两组手术时间、出血量、住院时间及术后并发症情况。结果观察组术中出血量少于对照组,住院时间明显短于对照组,差异均有统计学意义(P<0.05)。观察组出现胆汁瘘3例,对照组肺部感染4例,手术切口感染1例,经抗感染治疗后均痊愈。两组患者随访1年,均未发现有再生结石,也无胰腺炎发生。结论腹腔镜联合胆道镜治疗胆囊结石合并胆总管结石疗效显著,比传统的开腹手术更为安全有效,且住院时间短、术后恢复快,值得临床推广应用。
Objective To explore the clinical application value of laparoscopy combined with choledochoscopy in the treatment of cholecystolithiasis and eholedocholithiasis. Methods One hundred and two patients with eholecystolithiasis and choledocholithiasis in our hospital from September 2010 to October 2012 were randomly divided into control group（ n = 51 ）and observation group （n = 51 ）. The control group of patients was treated with laparotomy while the observation group was treated with laparoscopy combined with ehole- dochoscopy. The surgical time, bleeding volume,length of stay and postoperative complications were compared between the two groups. Results The bleeding volume and hospital time of the observation group were significant shorter than that of the control group （ P 〈 0. 05 ）. There were 3 cases of biliary fistula in the observation group while 4 cases of pulmonary infection and 1 case of surgical wound infection in the contro! group. After antimicrobial therapy, they were all recovered. No recycled stones were found neither in the obser- vation or the control group after one year follow-up. Conclusions The curative effect of the treatment with laparoscopy combined with choledochoscopy on cholecystolithiasis and choledocholithiasis was significant. Compared to traditional surgery, the technique has ad- vantages of safer, more effective, shorter hospital stay and faster recovery. Therefore it is worthy of clinical application.
