摘要
目的探讨自发性食管破裂的治疗效果。方法 40例自发性食管破裂患者,根据治疗方法不同分为手术组21例和支架组19例,比较两组治疗转归情况。结果食管自发性破裂至救治时间12 h以内病死率明显低于12 h以上(χ2=6.22,P<0.05),手术组病死率明显高于支架组(χ2=6.54,P<0.05)。结论应强调力争食管破裂时间12小时内救治的重要性,覆膜金属支架置入在自发性食管破裂治疗中比手术修补破口效果更确切可靠。
Objective To investigate the clinical effect of two surgical treatments on spontaneous esophageal rupture. Methods The clinical data of 40 cases with spontaneous esophageal rupture admitted in our hospital from January 2000 to January 2010 was retrospectively analyzed. The patients were divided into surgery group （n = 21 ） and all-covered metallic stent group （n = 19）. Clinical outcome of the two groups was comparatively analyzed comparatively. Results The time from esophageal spontaneous rupture to treatment within 12 hrs had a significantly lower mortality compared to over 12 hrs （X2 = 6. 22,P 〈 0. 05）. The mortacty rate of surgery group was significantly higher than that of the stent group （X2 = 6. 54,P 〈 0. 05 ）. Conclusions The treatment within 12 hours after e- sophageal rupture is very important. The clinical effect of implanting all-covered metallic stent on spontaneous esophageal rupture is better than that of surgery.
