摘要
目的总结内镜逆行胰胆管造影(endoscopic retrograde cholangiopancreatography,ERCP)相关并发症的类型、防治措施,提高ERCP的安全性。方法回顾性总结我院2008年7月至2012年6月297例患者ERCP相关并发症的类型和防治措施。结果 297例中,有5例患者不能配合或插管失败,排除在外,发生并发症22例,总发生率7.53%(22/292),其中急性胰腺炎11例(3.77%),胆管炎6例(2.06%),消化道出血3例(1.03%),消化道穿孔1例(0.34%),取石网嵌顿1例(0.34%)。1例因胆管炎合并恶性梗阻者因放弃治疗死亡,其余均经相应治疗好转或治愈。结论严格掌握ERCP的适应证及禁忌证,熟练规范的操作技巧,围手术期准备及处理,积极正确干预并发症,可提高ERCP的安全性。
Objective To analyze the causes,preventions and treatments of the complications related to endoscopic retrograde cholangiopancreatography（ERCP） in order to improve the safety of the technique. Methods Two hundred and ninety-seven cases trea- ted with ERCP in our hospital from July 2008 to June 2012 were retrospectively analyzed for causes, preventions and treatments of the complications related to ERCP. Results Five cases out of the total 297 cases were quitted because of failures in tolerance or cannula- tion in the operation. Total 22 cases suffered from complications （ 7.53 %, 22/292 ）. The complications included： 11 cases with acute pancreatitis （3.77 % ） ,6 cases with cholangeitis （2.06%）, 3 cases with duodenal hemorrhages （ 1.03 % ）, 1 case with duodenal perfora- tion（0.34% ）and lease with incarceration of stone grasping basket（0. 34% ）. One case with cholangeitis was dead for suffering from malignant biliary obstruction and abandoning further operation. The rest patients with complications were cured after corresponding treatment. Conclusions The safety of ERCP can be improved by controlling the indication and contraindication strictly, operating expertly and following rules, preparing and managing sufficiently during perioperations, treating actively and accurately.
