立体定向放射治疗同步吉西他滨治疗初治无法手术切除的胰腺癌32例 被引量:4

摘要 目的:探讨立体定向放射治疗同步吉西他滨治疗初治无法手术切除的胰腺癌患者的耐受性、安全性及疗效。方法:回顾性分析2008年8月至2011年8月32例初治无法手术切除的胰腺癌患者,采用立体定向放射治疗同步吉西他滨治疗。立体定向放射治疗:40%~60%等剂量曲线作为处方剂量,肿瘤直径>3cm,单次周边剂量3.0~3.5Gy,1次/d,5次/周,治疗次数为8~12次;肿瘤直径<3cm,单次周边剂量3.5~4.0Gy,1次/d,5次/周,治疗次数为8~10次。PTV周边照射总剂量为31.5~45Gy。化学治疗:吉西他滨1000mg/m2静滴d1、8。于放射治疗第1天开始,3周/周期,共4个周期。结果:32例患者均按计划完成放化疗,有效率为38.4%(12/32),中位生存期为12.4个月,1、2年生存率分别为35.0%和10.6%。主要毒副反应有胃肠道反应、血液学毒性和肝功能损害等,大多可耐受。结论:立体定向放射治疗同步吉西他滨治疗初治无法手术切除的胰腺癌疗效较好且安全,毒副反应能够耐受。
