摘要
目的总结腓动脉穿支链型供血皮瓣修复足踝部及小腿下段软组织缺损的手术方法及临床疗效。方法2008年9月至2010年9月,合肥医学院附属医院烧伤整形外科共收治的16例足踝部及小腿下段软组织缺损患者。男10例,女6例,年龄17～65岁,平均38岁。软组织缺损范围5 cm×4 cm～12 cm×6 cm;皮瓣面积为7 cm×5 cm～20 cm×9 cm,位于踝周及小腿下段15例,足跟负重区1例。病程1周至4月。结果术后16例皮瓣全部成活,创面及供区Ⅰ期愈合。12例患者获随访,随访时间6～18个月,平均12月。皮瓣色泽、质地及外形良好。皮瓣两点辨别觉8～12 mm。踝关节外观及功能恢复满意,行走不受影响。结论腓动脉穿支链状供血皮瓣血供可靠,操作简便,创面修复平整美观,肢体功能恢复满意,适合踝周及小腿下段小面积软组织缺损修复。
Objective To investigate the operative techniques and clinical results of flap supported by perforating branch of peroneal artery and catena -form blood vessel in repairing soft tissue defects in an- kle and inferior segment of leg. Methods From September 2008 to September 2010, 16 patients with soft tissue defects in ankle and inferior segment of leg were treated, including 10 males and 6 females, ran- ging in age from 17 years to 65 years ,with an average of 38 years. The range of soft tissue defects was from 5 cm ×4cm to 12cm ×6cm and the range of flap was from 7 cm ×5 cm to 20cm×9cm , the defects were in ankle areas and inferior segment of leg （ 15 cases） and weight - bearing heel （ 1 cases）. The time from injury to hospitalization was 7 days to 4 months. Results All the laps survived postoperatively, all the donor sites and wounds were healed by first intention. 12 patients were followed up 6 - 18 months （mean 12 months）. The texture, appearance and color of the flaps were good with two -point discrimi- nation of 8 - 12 mm. The function and shape of ankle obtained satisfactory recovery with normal in shoe gait. Conclusion With a reliable blood supply, simple operative procedure, and satisfactory recovery of limb function, the flap supported by perforating branch of peroneal artery and catena - form blood vessel is applicable for the repair of defects in the ankle and inferior segment of leg.
