摘要
目的观察不同病程糖尿病患者血清胱抑素C(CysC)水平的变化,以初步判断血清胱抑素C与糖尿病患者病程之间的关系。方法选取120例糖尿病患者,根据病程不同分为A组(病程<5年)、B组(病程5~10年)和C组(病程>10年),分别测量3组患者身高、体重、腰围、臀围,计算体质指数(BMI)、腰臀比(WHR),测定空腹血糖(FPG)、糖化血红蛋白(HbA1C)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C),血肌酐(Scr)、尿素氮(BUN)、尿酸(UA),血清胱抑素C(CysC)、24 h尿微量白蛋白(24 hMAU),观察各组相关资料的变化。结果 B组UA浓度较A组显著增高,C组Sr、CysC、24 hMAU浓度较B组显著增高(P<0.05),其余指标3组间无显著差异;CysC与病程(r=0.219,P=0.016))及尿蛋白(r=0.375,P=0.000)呈显著相关性。结论 2型糖尿病患者随着病程的延长,血清胱抑素C及24 h尿微量白蛋白明显增高,血清胱抑素C或可作为评价早期糖尿病肾病的指标。
Objective To observe the change of serum cystatin C (CysC) in different course of the dia- betes mellitus patients and preliminarily judge the relationship between serum CysC and the disease course of diabetic patients. Methods One hundred and twenty diabetic patients were divided into group A (less than 5 -year disease course), group B (between 5 and 10 years disease course) and group C (more than 10- year disease course). The height, weight, waistline, hipline, BMI, WHR, FPG, HbA1 C, TG, TC, LDL - C, HDL - C, Scr, BUN, UA, CysC and 24 hMAU among these three groups were detected. Results The serum UA concentration was in group B significantly higher than that in group A. The serum Sr, CysC and 24 hMAU contents in group C were higher than those in group B (P 〈 0.05 ). Single factor line analysis showed that the serum CysC levels were positively corrlated with di- abetes course (r =0. 219,P =0.016) and 24 hMAU (r =0. 375,P =0.000). Conclusion CysC and 24 hMAU increased significantly with the course of disease prolonged and CysC might be used as an e- valuation index of early diabetic nephropathy.
