摘要
目的 初步探讨眼冲击伤的致伤机理。方法 采用BST Ⅲ型生物激波管 ,建立以压力峰值(42 9± 38.8)kPa和 (834± 6 .4)kPa的轻、重两组伤型的眼冲击伤模型。在对不同强度的冲击波对眼组织的早期损伤的系列实验研究的同时 ,从临床损伤特点和生物力学角度讨论了眼冲击伤的致伤机理。结果 冲击波在眼部形成的眼内应力反射效应、应力剥落效应、压缩波与拉伸波的复合效应 ,可造成眼冲击伤特有的一些损伤特点。结论 眼冲击伤与眼钝挫伤的致伤机制有所不同。
Objective The manifestations of ocular posterior segment injuries by blast wave at the early stage and the mechanism of ocular blast injury were explored.Methods In this study,the BST-Ⅲbioshock tube was used to inflict ocular blast injury in rabbits.The peak pressure values for inflicting the mild and severe injuries were(429±38.8)kPa(8.9±1.8)ms and (834±6.4)kPa(6.6±1.7)ms,respectively.We used detection of CBF,ERG,FFA,and pathohistology.The early changes of tissue damage to ocular posterior segment(retina,choroid and optic nerve)were dynamically observed.Results At the early stage of ocular blast injury,every target of detections has marked difference.Conclusion The direct damage to retina,choroid and optic nerve,the reduction of CBF and retinal metabolic disorder caused by blast wave were the key factors of visual disfunction.Based on the clinical characteristics and from the view of biomechanics,it is suggested that the pathogenesis of ocular blast injury was different from that of ocular blunt injury.
