摘要
目的 通过实验研究和临床观察探讨脉冲电磁场 (Pulsedelectromagneticfields,PEMF)对断肢(指 )再植成活率的影响。方法 对 6 6只Wister大白鼠行股动、静脉端 -端吻合 ,术后PEMF刺激 ,并作血管通畅性和光镜、扫描电镜等检查。临床对 47例断肢 (指 )再植病人用PEMF刺激断肢 (指 ) (6 6肢 /指 ) ,再植后未行PEMF刺激的另外 89例 (112肢 /指 )作为对照组。结果 实验组血管通畅率显著增高 (P <0 .0 1) ,光镜及扫描电镜发现血管愈合加快。临床对照组 12肢 (指 )失败 ,10 0肢 (指 )成活 (89.3% ) ,PEMF组 2肢 (指 )失败 ,6 4肢 (指 )成活 (97.0 % ) ,显著高于对照组 (P <0 .0 5 )。结论 PEMF加快了血管愈合 ,可提高断肢 (指 )再植成活率。
Objective To explore the effect of pulse electromagnetic fields(PEMF) stimulation on replantation of amputated extremities or digits. Methods The rats and patients were stimulated by PEMF after vessels anastomosis. Results The long term patency rate of the vessels in the stimulated group was significantly higher than that of the control one (P< 0.01). Histological study revealed that the experimental group had slighter endovascular edema, minor inflammation and faster healing of the vessels. The scanning electron microscopy showed that the endothelial cells appeared earlier and aligned themselves parallel to the vessels at the anastomotic site in PEMF- treated rats. Forty seven patients (66 limbs or digits) who were treated by electromagnetic stimulation were succeeded except one(2 digits, survival rates was 97.0%). It was significantly higher than that (survival rates was 89.9%) in 89 no PEMF-treated patients (112 limbs or digits) (P<0.01). Conclusion It is considered that electromagnetic stimulation aggravates primary healing of the vessel, and thus promotes the long term patency rate.
