维持性血液透析患者血容量监测方法的比较研究 被引量:6

摘要 目的探讨在线血容量监测法与离心沉淀法监测血容量是否存在差异。方法应用德国Fresenius 4008S型血液透析机在线血容量监测系统(BVM),记录2010年1月至2011年12月在北京顺义区医院血液净化中心进行常规血液透析的76例尿毒症患者于透析开始、透析2 h和透析结束时相对血容量(RBV)值;同时根据离心沉淀法,根据计算公式ΔBV=(Hct后-Hct前)/Hct后分别计算出透析2 h和透析结束RBV变化(Δ'BV)(Hct后指的是透析2 h和透析结束时Hct值,Hct前指的是透析开始时Hct值),所得数据与在线血容量实际监测得出透析2 h和透析结束相对血容量变化ΔBV(计算公式为ΔBV=1-RBV后)(RBV后指透析2 h和透析结束时的RBV值)进行比较,比较两种检测方法是否存在差异。结果离心沉淀法计算Δ'BV值与在线血容量监测测得的ΔBV值比较,差异无统计学意义(P>0.05)。结论在线血容量监测法可以替代离心沉淀法监测血容量,是一种简单、无创、连续性的监测方法。 Objective To explore whether difference exists in results of relative blood volume between the blood volume monitor and centrifugal sedimentation technique. Methods Germany Fresenius 4008S hemedialysis machine on-fine blood volume monitoring system was used to record the relative blood volume (RBV) of pre-dialytie, two hours after dialytic and post dialytic of 76 patients. All patients were recruited from hemodialysis center of Shunyi Hospital from Jan. 2010 to Dec. 2011. At the same time, BV change (△'BV) was calculated by centrifugal sedimentation technique method : △ BV = ( Hct Later - Her Before) / Het Later( Het Later refers to Hct value after 2 h hemodialysis or at the end of hemodialysis, Her Before refers to Hct value at the beginning of hemodialysis). The obtained data and on-line blood volume monitoring blood volume change ( △ BV = 1 - RBV ) were then compared. Results RBV calculation of centrifugal precipitation change(△'BV) value and on-line blood volume monitoring measured BV values were compared,there was no significant difference( P 〉 0.05 ). Conclusion The BVM system, which is an uncomplicated, noninvasive and eontinous monitoring method.can take olace of centrifuaal sedimentation technique method.its.
