摘要
目的探讨常规脑电图(REEG)与动态脑电图(AEEG)在病毒性脑炎诊断中的辅助价值,提高早期诊断。方法收集医院神经科2010年6月-2012年6月收治的病毒性脑炎患者70例,将患者随机分为常规脑电图组(REEG)和动态脑电图组(AEEG),每组各35例,患者根据病情分为:轻度、中度和重度病毒性脑炎组。结果 REEG组发现异常18例,异常率为51.4%,AEEG组发现异常30例,异常率为85.7%,两组之间差异有统计学意义(P<0.05);REEG在轻度、中度和重度病毒性脑炎组中异常脑电图的检出率分别为30.0%、58.3%和61.5%,轻度与中、重度之间差异有统计学意义(P<0.05),而中和重度之间差异无统计学意义;AEEG在轻度、中度和重度病毒性脑炎组中异常脑电图的检出率分别为70.0%、83.3%和100.0%,各组之间差异有统计学意义(P<0.05),其中重度组中的检出率显著高于中度组(P<0.05),中度组中的检出率又显著高于轻度组(P<0.05)。结论脑电图(EEG)可以用于病毒性脑炎的早期辅助诊断,其中AEEG的临床应用价值优于REEG,值得临床推广应用。
OBJECTIVE To investigate routine(REEG) and ambulatory electroencephagraphy (AEEG) in the diagnosis of viral encephalitis so as to improve the early diagnosis. METHODS A total of 70 patients with viral encephalitis were enrolled in this study. The patients were divided into the REEG group and the AEEG group with 35 eases in each group. According to the severity of disease, each group was divided into the mild viral encephalitis group, the moderate viral encephalitis group, and severe viral encephalitis group with 10, 12, and 13 cases in each group. RESULTS In the REEG group, 18 cases were found abnormal with the abnormal rate of 51.4%. In the AEEG group, 30 cases were found abnormal with the abnormal rate of 85.7%, the difference between the two groups was statistically significant (P〈0.05). The detection rates of abnormal REEG in the mild, moderate and severe viral encephalitis groups were 30.0%, 58.3%, and 61.5%, respectively , the difference between the mild group, the moderate group, and the severe group was statistically significant (P〈0.05), however, the difference between the moderate group and the severe group was not statistically significant. The detection rates of abnormal AEEG in the mild, moderate and severe viral encephalitis group were 70.0%, 83.3%, and 100.0%, respective- ly, the difference between the three groups was statistically significant (P〈0.05), the detection rate of the severe group was significantly higher than that of the moderate group(P〈0.05), and the detection rate of the moderate group was significantly higher than that of the mild group(P〈0.05). CONCLUSION The EEG can be used in the early adjuvant diagnosis of viral encephalitis, the value of AEEG is better than that of REEG in the clinical appli- cation ,and it is worthy to be promoted in the hospital.
