支持向量机模型预测前哨淋巴结阳性乳腺癌非前哨淋巴结状态的研究 被引量:1

摘要 目的研究支持向量机模型(SVM)预测前哨淋巴结(SLN)阳性乳腺癌非前哨淋巴结状态的准确性、可行性。方法回顾性收集201例SLN阳性乳腺癌患者的临床病理资料,包括患者年龄、肿瘤大小、组织学类型、组织学分级、有无脉管侵犯、雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(CerbB2)、阳性SLN大小、阳性SLN数量、阴性SLN数量、阳性SLN包膜外侵犯情况。采用统计过滤结合模型依赖性筛选的方法选取特征向量,选出P值最小的任意组合用于SVM的输入,用10倍交叉验证评估模型的预测效果,建立训练模型。重新收集85例SLN阳性乳腺癌患者,作为盲法测试集,加入模型进行测试,计算模型盲法测试的敏感度、特异度。结果 SVM筛选出建立的模型交叉验证准确率最高的组合为:阳性SLN数量、阳性SLN大小、阴性SLN数量、阳性SLN包膜外侵犯。用这4个变量建立的模型训练准确性为92.0%,交叉验证准确性为87.6%,85例盲法测试模型准确性为91.8%。结论 SVM预测SLN阳性乳腺癌非前哨淋巴结状态是准确、可行的,有助于指导临床治疗。
