骨髓小粒拉片细胞学检查在血液病诊断中的临床应用 被引量:2

摘要 目的研究骨髓小粒拉片(简称拉片)细胞形态学检查对血液病的诊断价值。方法收集四川省绵阳市人民医院2008年1月~2012年8月血液系统疾病患者83例,取患者的骨髓小粒制备拉片,以同期同部位采集的骨髓穿刺涂片(简称涂片)和骨髓活检切片(简称切片)为对照,进行细胞形态学检查。结果在增生程度的判断上,以切片为金标准,拉片与切片差异无统计学意义(χ2=0.9051,P>0.05),拉片显著高于涂片(χ2=6.1179,P<0.05);临床诊断符合率以拉片最高为86.7%,显著高于涂片(χ2=7.8253,P<0.01),但与切片差异无统计学意义(χ2=0.4238,P>0.05),三种方式联合检测诊断符合率达94.0%。结论骨髓小粒拉片在增生程度的判断上及低增生性血液系统疾病的诊断上有明显优势,可作为骨髓涂片形态学检查的补充,二者联合切片检查,能提高血液病诊断的准确性。 Objective To explore the cytology examination of fragments smear in diagnosis for hematopathy.Methods 83 cases with hematopathy were collected from January 2008 to August 2012 of Mianyang People ′s Hospital.The patient’s bone marrow fragments were prepared for fragments smear.At the same time,the aspirate of the same part was prepared for bone marrow smear and bone marrow section for control.Fragments smear,bone marrow smear and bone marrow section respectively carried out cytology examination.Results Biospy as a golden standard,there was no significant difference between fragments smear and bone marrow section(χ2=0.9051,P &gt; 0.05).The accuracy of fragments smear was significantly higher than bone marrow smear(χ2=6.1179,P &lt; 0.05).The highest clinical diagnosis rate was fragments smear(86.7%),significantly higher than the bone marrow smear(χ2=7.8253,P &lt; 0.01),no statistical significance compared with bone marrow sections(χ2=0.4238,P &gt; 0.05).The diagnosis rate combined three ways was 94.0%.Conclusion Fragments smear have obvious advantages in the judgment of the degree of proliferation and the diagnosis of low proliferative hematopathy.Fragments smear can be supplemented as cytology examination of bone marrow smear.The combination examination of fragments smear,bone marrow smear and bone marrow section can improve the diag nostic accuracy of the hematopathy.
