摘要
目的探讨局部晚期非小细胞肺癌(NSCLC)同步放化疗和序贯放化疗的临床疗效。方法对收治的128例局部晚期非小细胞肺癌患者的临床资料进行回顾性分析,根据治疗方法分为两组,其中同步放化疗组62例,序贯放化疗组66例。比较两组患者的近期疗效、生存率、不良反应发生率及生活质量。结果同步放化疗组和序贯放化疗组的治疗有效率分别为63.6%和41.9%,差异有统计学意义(P<0.05)。同步放化疗组1年和3年生存率显著高于序贯放化疗组(P<0.05);同步放化疗组III～IV级白细胞减少患者显著高于序贯放化疗组(P<0.05);两组患者在放射性肺损伤和食管炎方面差异无统计学意义(P>0.05)。治疗后1个月同步放化疗组生活质量核心问卷(QLQ-C30)躯体功能和总健康状况评分显著优于序贯放化疗组(P<0.05)。结论与序贯放化疗相比,同步联合放化疗治疗局部晚期小细胞肺癌可提高临床疗效,患者生活质量逐渐提高。
Objective To explore the clinical effect of concurrent chemo-radiotherapy and sequential chemo-radiotherapy in treatment of locally advanced non-small cell lung cancer.Methods The data of 128 cases with locally advanced non-small cell lung cancer were retrospectively analyzed,including 62 patients undergoing concurrent chemo-radiotherapy and 66 cases treated with sequential chemo-radiotherapy. Short term effect,survival rate,toxicity reaction occurrence rate and life quality were compared between two groups.Results The effective rates of treatment were 63.6% and 41.9% in concurrent chemo-radiotherapy group and sequential chemo-radiotherapy group（P 0.05）;1 and 3 years survival rates in concurrent chemo-radiotherapy group were much higher than those in sequential chemo-radiotherapy group（P 0.05）; Ⅲ ～ Ⅳ grade leukopenia in concurrent chemo-radiotherapy group greatly increased（P 0.05）,while there were no significant difference of radiation-induced lung injury and esophagitis between two groups（P 0.05）;Physical function and total health status scores of QLQ-C30 after treatment in concurrent chemo-radiotherapy group were significant higher than those in sequential chemo-radiotherapy group（P 0.05）. Conclusions Comparison with sequential chemo-radiotherapy,concurrent chemo-radiotherapy for treatment of locally advanced non-small cell lung cancer can greatly increase the clinical effect and life quality.
