摘要
通过对1例发热伴皮疹的老年患者实施的药学监护过程的观察。临床药师参与临床诊疗,直接对患者进行药学监护,提出合理化用药建议,探讨临床药师的工作切入点,从而提高医疗质量及患者的用药依从性。通过药学监护,充分体现出药师在医疗团队(医师、药师、护师、营养师、心理咨询师)中的重要作用。
To observe the effect of pharmaceutical care on one case of elderly patient with fevers and rashes.Clinical pharmacists can provide pharmaceutical care and rationalization suggestion of drug,and solve clinical problems to discuss the working pattern and key points,in order to improve Medical quality and patients＇ medication adherence.The roles of Clinical pharmacists in medical teams （including doctors,pharmacists,senior nurse,dietitian and psychological consultant） become important by promoting pharmaceutical care.
