摘要
目的观察开腹手术和腹腔镜手术治疗儿童急性阑尾炎的临床效果及预后。方法回顾性分析2006年10月至2012年10月采用开腹手术(开腹组,n=100)和腹腔镜手术(腹腔镜组,n=100)治疗的200例急性阑尾炎患儿的临床资料。结果开腹组的术中出血量、术后进食时间、疼痛持续时间、肛门排气时间和住院时间都显著多于腹腔镜组,差异均有统计学意义(P<0.05),但是开腹组与腹腔镜组的手术时间差异无统计学意义(P>0.05)。开腹组镇痛剂使用、切口感染、术后瘢痕、腹腔脓肿和粘连性肠梗阻的患者例数都显著多于腹腔镜组,差异均有统计学意义(P<0.05)。结论与开腹手术比较,腹腔镜手术治疗儿童急性阑尾炎具有安全、创伤小、恢复快、住院时间短、美观及并发症少等优势,可以作为治疗儿童急性阑尾炎的首选方法。
Objective To compare and evaluate the clinical effect and prognosis of open surgery versus laparoscopy for treating acute appendicitis in children. Methods The clinical data of 200 children patients with acute appendicitis treated by open surgery （open surgery group, n=100） or laparoscopy （laparoscopy group, n=100） in our hospital from October 2010 to October 2012 were analyzed retrospectively. Results The pefioperative bleeding, the meal time after sugery, pain time, anorectum exhaust time, length of hospital stay in the open surgery group were significant higher than those in the laparoscopy group. However, there is no statistically significant difference in operation time between the two groups. In addition, the numbers of the analgesic, infection of incisional wound, slight scars after operation, intrapefitoneal abscess and adhesive ileus in the open surgery group were significantly higher than those in the laparoscopy group. Conclusion Compared with open surgery, the laparoscopy for treating acute appendicitis in children has a lot of advantages, including safety, minimal invasion, early recovery, short length of hospital stay, and few complications, which can be used as preferred method to treat children with acute appendicitis.
