臭氧联合胶原酶注射治疗腰椎间盘突出症668例疗效分析 被引量:4

摘要 目的通过对小C臂导向下臭氧与胶原酶联合注射治疗腰椎间盘突出症患者的疗效观察与分析,总结临床经验,提高疗效。方法对在我院行小C臂导向下臭氧与胶原酶联合注射治疗1年以上的870例患者进行电话随访,共随访到668例,根据治疗后的时间分为三组:其中Ⅰ组(治疗后12~24个月)238例、Ⅱ组(治疗后25~48个月)230例、Ⅲ组(治疗后49~72个月)200例,采用改良的Macnab腰腿痛疗效评定标准评估疗效。结果Ⅰ组优良率为91.18%;Ⅱ组优良率为90.87%;Ⅲ组优良率为89.00%,总体优良率为90.35%。三组优良率比较差异无统计学意义(P>0.05)。所有患者均无重大不良事件和严重并发症发生。结论小C臂导向下臭氧与胶原酶联合注射治疗腰椎间盘突出症新技术安全,远期疗效满意。术前明确诊断,严格把握适应证;术中准确定位,熟练穿刺技术,严格操作规程,实施个性化方案和术后规范康复训练是提高远期疗效、减少并发症的关键所在。 Objective To observe and analyze the clinical effect of injection of ozone and collagen enzyme in the treatment of lumbar intervertebral disc herniation under the guidance of C-arm X-ray. Methods A total of 870 patients were injected with ozone and collagen enzyme for more than one year, 668 of which were followed up by telephone interview. According to the time after treatment, the patients were divided into three groups: Group Ⅰ (n=238, 12-24 months after treatment), Group Ⅱ (n=230, 25-48 months after treatment), Group Ⅲ (n=200, 49-72 months after treatment). The clinical efficacy was assessed by modified Macnab criteria. Results The excellent and good rate was 91.18% in group Ⅰ, 90.87% in group Ⅱ, 89.00% in group Ⅲ, with no statistically significant difference between the three groups (P〉0.05). The overall excellent and good rate was 90.35%. The patients had no significant adverse reactions and serious complications. Conclusion In the treatment of lumbar intervertebral disc herniation, injection of ozone and collagen enzyme results in satisfactory long-term efficacy. Preoperative diagnosis, accurate intraoperative positioning, skilled puncture technique, strict operating procedures, individual therapy and standard postoperative rehabilitation are the key to improve the long-term efficacy.
