摘要
目的探讨6%羟乙基淀粉(万汶)在感染性休克容量治疗中的有效性和安全性。方法采用前瞻性研究方法,按照区组配对原则选取我院2010年6月～2011年4月间重症监护病房(ICU)收治的52例感染性休克患儿作为研究对象。对照组予乳酸林格氏液,观察组予6%羟乙基淀粉(万汶)进行复苏。对两组APTT、PT、FIB\D-二聚体、血乳酸、APACHEⅡ、TNF-α、IL-10等指标进行比较。结果治疗前和治疗后24小时,APTT、PT、FIB\D-二聚体等指标差异无统计学意义(P>0.05)。治疗后,治疗组血乳酸、APACHEⅡ、TNF-α、IL-10分别为(5.8±1.2)mmol/L、(20.9±2.6)pg/mL、(103.3±10.2)pg/mL和(11.9±2.6)pg/mL,明显低于对照组的(6.9±1.3)mmol/L、(26.4±3.9)pg/mL、(114.5±17.6)pg/mL和(14.2±2.9)pg/mL,差异有统计学意义(P<0.05)。结论 6%羟乙基淀粉(万汶)在感染性休克的容量治疗中安全有效,可降低炎性反应,具有重要的临床应用价值。
Objective To expore efficacy and safety on 6% hydroxyethyl starch in the treatment of septic shock. Methods By prospective study method, According to the random matching principle, 52 cases patients with septic shock of intensive care unit （ICU） were as the research object from June 2010 to April 2011, the control group was given by lactate ringer/s solution, the observation group was given 6% hydroxyethyl starch for recovery. AFFF, PT, FIB, D-dimer, blood lactic acid, APACHE I1, TNF-α and IL-10 of two groups were compared. Results Before and iffter 24 hours of treatment, AFFF, PT, FlB,D-dimer were compared, there was no statistically significant difference （P 〉 0.05）. After treatment, the treatment group of blood lactic acid, APACHE H ,TNF-α, IL-10 were （5.8±1.2）mmol/L, （20,9±2.6）pg/mL, （103.3±10.2） pg/mL, （11.9±2.6） pg/mL, significantly lower than the contrast of （6.9±1.3）mmol/L, （26.4±3.9） pg/mL,（l14.5±17.6） pg/mL, （14.2±2.9） pg/mL, the difference was statistically significant （P 〈 0.05）. Conclusion The 6% hydroxyethyl starch for treatment of septic shock is safe and effective, and reduce inflammatory reaction, it has the important clinical application value.
