摘要
目的探讨鼻咽癌(NPC)患者Survivin、Bcl-2、Pml基因的表达情况,分析其与NPC临床分期及预后的关系。方法选取2010年1月～2012年5月本院与合作医院耳鼻喉科门诊收治的55例NPC患者和50鼻咽慢性炎症患者,将其作为临床研究对象。采用实时荧光定量PCR检测组织和外周血中Survivin、Bcl-2和Pml mRNA的表达,同时利用TUNEL法检测细胞的凋亡情况。结果 NPC患者Survivin、Bcl-2、Pml基因表达阳性率分别为68.00%、74.00%、60.00%,均明显高于对照组;Ⅰ、Ⅱ期NPC患者Survivin、Bcl-2基因表达阳性率明显低于Ⅲ、Ⅳ期NPC患者,而Pml基因表达阳性率则明显高于Ⅲ、Ⅳ期NPC者;Survivin、Bcl-2、Pml阳性率患者放疗后1年后的生存率明显低于6个月后,差异具有统计学意义(P<0.05)。结论 NPC患者Survivin、Bcl-2和Pml基因表达呈高阳性率,且Survivin、Bcl-2、Pml基因的表达与临床分期和预后存在密切的相关性,临床上可作为预测NPC放射敏感性和预后的有效指标。
Objective To investigate the gene expression of Survivin, Bcl-2, Pml and its relationship with clinical stage and prognosis of nasopharyngeal carcinoma. Methods Fifty nasopharyngeal carcinoma patients who were treated in the ear-nose-throat department of cooperation hospital and our hospital from January 2010 to May 2010 were chosen as the research objects. The expression of Survivin, Bcl-2 and Pml mRNA in tissue and peripheral blood were detected by real-time fluorescent quantitative PCR, at the same time, the cell apoptosis were detected by the TUNEL method. Results The expression positive rate of Survivin, Bcl-2, Pml gene was 68.00%, 74.00%, 60.00%, respectively, they were significantly higher than the control group （P 0.05）. The expression positive rate of Survivin, Bcl-2, Pml gene in Ⅰ and Ⅱ period nasopharyngeal carcinoma patients were lower than that Ⅲ and Ⅳ period nasopharyngeal carcinoma patients （P 0.05）. Survival rates in six month were lower than that three months in with nasopharyngeal carcinoma patients expression positive rate of Survivin, Bcl-2, Pml, the difference was statistically significant （P 0.05）. Conclusion The expression positive rate of Survivin, Bcl-2, Pml gene in the nasopharyngeal carcinoma patients is high, the expression positive rate of Survivin, Bcl-2, Pml gene is close correlation with clinical stage and prognosis of nasopharyngeal carcinoma, they can be used as a clinical effective predictor of radiosensitivity and prognosis of nasopharyngeal carcinoma.
