消癌顺气汤联合CAF化疗对肝郁气滞型乳腺癌患者血管内皮生长因子及T细胞亚群的影响 被引量:4

摘要 目的观察消癌顺气汤联合CAF化疗对肝郁气滞型乳腺癌患者血管内皮生长因子(VEGF)及T细胞亚群的影响。方法将90例肝郁气滞型乳腺癌患者随机分为3组,化疗组30例手术前予CAF化疗治疗,治疗组30例手术前予消癌顺气汤联合CAF化疗,未化疗组30例手术前未进行化疗治疗及其他特殊治疗。观察3组疗效,比较3组治疗前后VEGF水平变化,并对患者治疗前VEGF与相关因素进行分析,比较3组治疗后癌组织VEGF阳性数、VEGF阳性表达的积分光密度(IOD)值及肿瘤微血管密度(MVD),比较3组手术前后CD4、CD8及CD4/CD8变化。结果化疗组、治疗组进展(PD)率低于未化疗组(P<0.01),总有效率高于未化疗组(P<0.05);治疗组PD率低于化疗组(P<0.05)。治疗组治疗后VEGF水平较本组治疗前、未化疗组治疗后均降低(P<0.05)。患者治疗前VEGF水平与肿瘤直径大小、是否存在淋巴结转移、TNM分期均呈正相关,与患者年龄无相关(P>0.05)。治疗组治疗后MVD、VEGF阳性表达的IOD值较化疗组、未化疗组低(P<0.05)。治疗组、未化疗组手术后CD4、CD4/CD8较化疗组手术后升高(P<0.05);化疗组手术前后CD4、CD4/CD8比较差异均有统计学意义(P<0.05)。结论消癌顺气汤联合CAF化疗对肝郁气滞型乳腺癌患者VEGF有明显的抑制作用,对患者化疗前后的T细胞亚群有一定的保护作用。 Objective To explore Xiaoaishunqi decoction on vascular endothelial growth factor and T cell subsets in breast cancer patients with stagnation of liver Qi. Methods A total of 120 breast cancer patients with stagnation of liver Qi were randomly divided into 3 groups, group A was given conventional chemotherapy before oper- ation, group B was given conventional chemotherapy and Xiaoaishunqi decoction oral, group C was not given conven- tional chemotherapy and other special treatment before operation. The levels of VEFG were observed before and after treatment in three groups. VEGF positive number of tumor tissue, VEGF positive expression of integrated optical den- sity (IOD) value and tumor microvessel density (MVD) were compared after treatment. CD4, CD8 and CD4/CD8 were compared before and after operation in three groups. Results The rate of progression in group A and B was lower than that in group C ( P 〈 0. O1 ). The total effective rate in group A and B was higher ( P 〈 O. 05 ). The level of VEGF after treatment in group B was decreased as compared with that before treatment and after treatment in group C ( P 〈 O. 05). The level of VEFG before treatment was positive correlation with tumor diameter, the existence of lymph node metastasis and TNM stage. MVD, VEGF positive expression of IOD value in group B was lower than that in group A and C ( P 〈 0.05). There was significant difference between before and after treatment in group A on CD4 and CD4/CD8 ( P 〈 0.05). Conclusions Xiaoaishunqi decoction combined with conventional chemotherapy has obvious inhibition on VEGF in breast cancer patients with stagnation of liver Qi, and protection on T cell subsets before and after chemotherapy.
