关节镜下缝合固定治疗膝关节外侧半月板附着部松弛疗效观察 被引量:7

Clinical Study of Suture Repairing Loose Lateral Meniscus of the Knee Under Arthroscopy

摘要 目的观察关节镜下缝合固定治疗膝关节外侧半月板附着部松弛的术后疗效。方法对2006年3月—2012年6月我院收治的10例经术中探查膝关节外侧半月板附着部位松弛的患者在关节镜下进行半月板缝合固定,观察术后治疗效果并进行随访,术前及术后采用Lysholm评分、特种外科医院膝关节评分(HSS)对手术疗效进行评价。结果 10例膝关节中前角松弛6例,体部松弛3例,前角合并体部松弛1例,未发现后角松弛患者;3例膝关节外侧半月板外侧边缘与外侧关节囊相邻部位退变,2例膝关节内侧半月板向外侧移位,1例膝关节内侧半月板内侧边缘退行性改变。所有患者术后无感染、关节僵硬、神经损伤及软组织坏死等并发症。患者平均随访18个月,患者术后膝关节Lysholm评分及HSS〔(83.5±12.6)分,(89.0±7.3)分〕均高于术前〔(32.6±14.6)分,(40.0±8.6)分〕,差异有统计学意义(t值分别为2.882和2.956,P<0.01)。结论对松弛的膝关节半月板附着部位进行缝合固定,效果满意,有利于恢复半月板正常功能,延缓骨性关节炎的发生与发展。 Objective To study the effect of suture repairing loose root of lateral meniscus of knee under arthrosco- py. Methods 10 cases of loose lateral meniscus of the knee admitted to our hospital from March 2006 to June 2012 were sutured under arthroscopy. The curative effect of meniscal suture was observed by postoperative follow - up and the Lysholm knee score and HSS. Results Among the ten cases, 6 had loose anterior angle, 3 had loose body and one had loose anterior angle combined with loose body and no loose posterior angle was found. The adjoining part between the outer edge of lateral meniscus and lateral joint capsule were degenerative in 3 cases, 2 cases had lateral direction migration of medial meniscus and one case had degenera- tive changes in the medial edge of medial meniscus. No infection, joint stiffness, nerve injury and soft tissue necrosis were found among the patients. The patients were followed up for an average of 18 months. The Lysholm score and HSS after operation [ (83.5 ± 12.6) and (89.0 ± 7.3) ] were significantly higher than those before operation [ ( 32. 6 ± 14. 6) and (40.0 ± 8.6) ] ( t = 2. 882, 2. 956, P 〈 0. 01 ) . Conclusion Repairing the loose root of meniscus of the knee helps maintain the normal function of the meniscus and could delay the occurrence and development of osteoarthritis.
