摘要
目的探讨2型糖尿病(T2DM)患者的血液流变学指标与血糖监测指标的相关性及其在T2DM微血管病变中的作用。方法选取2011年1月—2012年9月在本院住院的116例T2DM患者(T2DM组)和60例体检健康者(对照组),检测其空腹血糖(FPG)、糖化血红蛋白(HbA1c)和血液流变学指标,并分析血液流变学指标与血糖监测指标的相关性。结果 T2DM组患者FPG、HbA1c、全血低切黏度、全血中切黏度、全血高切黏度、血浆黏度、血细胞比容、红细胞聚集指数、红细胞变形指数、红细胞刚性指数均高于对照组,差异有统计学意义(P<0.05)。相关分析显示,T2DM患者全血低切黏度、全血中切黏度、全血高切黏度、血浆黏度、红细胞聚集指数、红细胞变形指数、红细胞刚性指数与FPG(r值分别为0.790、0.782、0.804、0.650、0.803、0.755和0.649,P均<0.01)、HbA1c(r值分别为0.273、0.217、0.297、0.226、0.226、0.441和0.269,P均<0.05)均呈正相关。结论 T2DM患者血液流变学指标与血糖监测指标呈正相关,故常规检测血液流变学指标对T2DM患者微血管病变的病情预测、早期诊治和疗效观察有重要意义。
Objective To investigate the correlation of blood rheology and blood glucose monitoring indicators in pa- tients with type 2 diabetes mellitus (T2DM) and their application in type 2 diabetic patients with microangiopathy. Methods Fasting plasma glucose (FPG), glycosylated hemoglobin (HbA1c) and blood theology of 116 T2DM patients (T2DM group) and 60 healthy people ( control group) admitted to our hospital from January 2011 to September 2012 were detected to analyze the correlation of blood rheology and blood glucose monitoring indicators. Results The FPG, HbA1c, low shear viscosity of whole blood, middle shear viscosity of whole blood, high shear viscosity of whole blood, plasma viscosity, hematocrit, erythrocyte ag- gregation index, erythrocyte deformation index and erythrocyte rigidity index were all significantly higher than those of the control group (P 〈 0. 05) . Correlation analysis showed that low shear viscosity of whole blood, middle shear viscosity of whole blood, plasma viscosity, high shear viscosity of whole blood, erythrocyte aggregation index, erythrocyte deformation index and erythro- cyte rigidity index were all positively correlated with FPG (r = 0. 790, 0. 782, 0. 804, 0. 650, 0. 803, 0. 755 and 0. 649, P all〈0.05) andHbA1c (r=0.273, 0.217, 0.297, 0.226, 0.226, 0.441 and0.269, Pall〈0.05) .Conclusion Hemo- rheology indexes is positively correlated with blood glucose monitoring indicators in patients with T2DM, thus it is of great impor- tance for the prediction of disease, early diagnosis and curative effect observation in T2DM patients with microangiopathy.
