摘要
目的探讨1型糖尿病患者骨密度(BMD)变化及相关影响因素。方法收集我院就诊的1型糖尿病患者47例,对照组为40例非糖尿病健康人群。体格检查计算腰臀比(WHR)及体质指数(BMI),葡萄糖氧化酶法检测空腹血糖(FPG)及餐后2 h血糖(2 hPG)、高效液相法检测糖化血红蛋白(HbA1c)、放免法检测空腹C肽(FCP)及餐后2 hC肽(2 hCP)水平,双能X线骨密度仪测定腰椎(L2-L4)及左股骨颈BMD。结果两组间年龄差异无统计学意义(P>0.05),1型糖尿病组BMI、WHR、FCP、2 hCP、腰椎及股骨颈BMD低于对照组,但FPG、2 hPG及HbA1c高于对照组,差异有统计学意义(P<0.05)。糖尿病病程、HbA1c与腰椎及股骨颈BMD呈负相关(P<0.05),FCP、2 hCP与腰椎及股骨颈BMD呈正相关(P<0.05)。进行多因素回归分析发现,进入方程的为病程、HbA1c、FCP及2 hCP(P<0.05)。结论 1型糖尿病患者BMD低于健康人群,1型糖尿病患者的病程、HbA1c、FCP、2 hCP水平与BMD关系密切。
Objective To explore the bone mineral density (BMD) in patients with type 1 diabetes mellitus and its as- sociated factors. Methods Forty - seven patients with type 1 diabetes mellitus treated in our hospital were assigned as the study group, and 40 healthy people as the control group. Waist- to- hip ratio (WHR) and body mass index (BMI) were calculated according to physical examination results. Fasting plasma glucose (FPG) and 2 hour postprandial glucose (2 hPG) were detected by glucose oxidase method, HbA1C was tested by high efficiency liquid chromatography, fasting C -peptide (FCP) and 2 hour postprandial C - peptide (2 hCP) levels by radio immunoassay, and BMD of lumbar ( L2 - L4 ) and left femoral neck by DXA. Results No difference in age was observed between the two groups ( P 〉 0.05 ). The study group was lower in BMI, WHR, FCP, 2 hCP, and BMD of lumbar and left femoral neck, but higher in FPG, 2 hPG, and HbAlo than the control group (P 〈 0. 05). The disease course and HbA1C were negatively, while FCP and 2 hCP were positively correlated with BMD of lum- bar and left femoral neck (P 〈 0. 05). Multiple regression analysis showed correlation of the diabetes course, HbA1C, FCP, and 2 hCP with BMD of lumbar and left femoral neck ( P 〈 0. 05) . Conclusion BMD of patients with type 1 diabetes mellitus is lower than that of healthy people. The disease course, HbA1C, FCP, and 2 hCP are correlated with BMD of lumbar and left fem- oral neck in patients with type 1 diabetes mellitus.
