摘要
目的探讨初次旋转铰链膝关节置换术治疗严重畸形的膝关节病变的近期疗效。方法采用Endo-Model旋转铰链假体行初次膝关节置换患者30例(30膝),比较手术前后X线片、KSS临床及功能评分、膝关节活动范围以及屈曲角度。结果患者手术时间平均(85.0±14.2)min。无一例发生感染、血栓、神经血管损伤等并发症。所有患者均获得随访,随访时间18~48个月,平均33个月。膝关节活动范围、屈曲角度、KSS临床和功能评分均大于术前(P<0.01)。术后膝关节胫股关节对线及髌股关节对线较术前改善,差异有统计学意义(P<0.01)。9例(30%)膝前痛患者髌骨轴位片显示髌骨有移位或倾斜。结论 Endo-Model旋转铰链膝关节假体早期效果满意,是手术治疗膝关节严重畸形病变的一个良好选择,但应重视髌股关节并发症的发生。
Objective To explore the short-term clinical outcome of primary rotating hinge knee arthroplasty for knees with severe deformity. Methods A retrospective study of the clinical outcomes was done in 30 knees of 30 patients after receiving the total knee arthroplasty (TKA) with the Endo-Model rotating hinge prostheses. All patients were evaluated preoperatively and postoperatively using X-ray image, the Knee Society Score (KSS), the flexion, the femorotibial angle and fine/excellent rate of patellofemoral alignment. Results The mean operation time was (85.0± 14.2) rain. No complications of infection, thrombosis and neurovascular injury were found in patients. All patients were followed up, and the average fol- low-up time was 33 months (18-48 months). The knee flexion and extension range of motion (ROM), the KSS clinical score and function score were improved after operation (P 〈 0.01). The femorotibial angle and fine/excellent rate of patellofemoral alignment were improved after operation as well (P 〈 0.01). There were 9 patients having anterior knee pain and showed patel- lars shift or tilt in the axial view of X-ray. Conclusion The short-term clinical outcomes of Endo-Model rotating hinge TKA are ideal, and which is a good therapeutic choice for knees with severe deformity. However, the high rate of patellofemoral joint complications should be encountered.
