摘要
目的探讨整合素α3(integrin a3)和环氧合酶-2(cyclooxygenase-2,COX-2)在结直肠癌组织中的表达及其与临床病理特征的关系,评估其在结直肠癌诊断、预后及治疗的价值。方法收集笔者医院结直肠癌病人标本30例及其相应癌旁正常组织,通过免疫组化和实时荧光定量聚合酶链反应(real-time PCR)检测整合素α3和COX-2蛋白和mRNA表达情况,并分别将其与临床病理特征的关系进行统计学分析。结果整合素α3蛋白和mRNA在结直肠癌组织较癌旁正常组织表达明显降低(P=0.007和P<0.001),COX-2蛋白和mRNA在结直肠癌组织较癌旁正常组织表达均明显升高(P=0.005和P<0.001);且整合素α3和COX-2两者mRNA表达与TNM分期、性别、年龄、肿瘤大小、分化程度、部位组间不存在显著性差异(P>0.05);整合素α3蛋白表达水平与TNM分期组间存在显著性差异(P=0.014),整合素α3和COX-2蛋白及mRNA水平均与淋巴结转移有关(P<0.05);COX-2及整合素α3蛋白表达水平与患者与分化程度、肿瘤大小呈相关性(P<0.01);同时整合素α3和COX-2的mRNA表达在结直肠癌组织中的表达具有相关性(P=0.029,r=-0.398)。结论结直肠癌中的整合素α3表达低于癌旁正常组织而COX-2表达高于癌旁正常组织,且两者表达具有一定相关性。COX-2和整合素α3与结直肠癌的浸润和转移密切相关,可以作为诊断结直肠癌的一种标志物并有可能估计患者的预后。
Objective To investigate the expression of COX - 2 （Cyclooxygenase - 2） and integrin α3 and the correlation with the clinical pathological factors of colorectal carcinoma, and evaluate their value in diagnosis, treatment and prognosis of colorectal carcinoma. Methods Immunohistochemical assay and real -time reverse transcription -polymerase chain reaction （RT -PCR） were used to detect respectively proteins and mRNA expression of the COX - 2 and integrin α3 gene both in 30 colorectal carcinoma samples and matched tumor - adjacent tissue samples. The correlations of the expression of integrin α3 and COX - 2 with clinical and pathological parameters in colorectal carcinoma were then analyzed respectively. Results The expression levels of proteins and mRNA of COX - 2 and integrin α3 between tumor tissues and matched tumor - adjacent tissue were detected to be significant different （P = 0. 005 ,P = 0. 007,P 〈 0. 001 and P 〈 0. 001,respectively）. The mRNA expression of COX -2 and integrin α3 were not associated with age, sex, tumor size, tumor differentiation and clinical stage （ P 〉 0.05 ）. The proteins expression of integrin α3 mRNA expression were associated with clinical stage （ P = 0. 014）. And the expression of both the mRNA and proteins of COX -2 and integrin α3 were associated with lymph node metastasis （P 〈 0.05）. The proteins expression of COX - 2 and integrin α3 were associated with tumor size and tumor differentiation （ P 〈 0.01 ）. There was negative correlation of mRNA expression of COX - 2 and integrin α3 （ r = - 0. 398, P = 0. 029）. Conclusion The expressiones of proteins and mRNA of COX - 2 gene in colorectal carcinoma are both higher than those in tumor - adjacent tissue,and the integrin ot3 is lower. COX -2 and integrin α3 may exhibit a high cross correlation. The expressiones of integrin 0% and COX -2 were highly associated with the metastasis and infiltration in colorectal carcinoma,which may be helpful in judging diagnosis and prognosis of colorectal carcinoma.
