摘要
目的:探讨低电压、低碘对比剂剂量(双低)头颈CTA的可行性。方法:30例患者行双低头颈CTA检查,对比剂剂量为0.3mL/kg,管电压100kVp。30例作为对照组行常规CTA检查,对比剂剂量为60mL,管电压120kVp。采用独立样本t检验比较两组的对比剂剂量、剂量长度乘积(DLP)、体型特异性剂量估计值(SSDE);采用Mann-Whitney U检验比较两组图像质量等级、头颈动脉各段的CT值等级。结果:双低组对比剂剂量均值为(28.13±2.76)mL,对照组为60mL,两组差异有统计学意义(P<0.05)。双低组SSDE均值为(24.29±0.98)mGy,对照组为(31.77±2.27)mGy,差异有统计学意义(P<0.05)。双低组DLP均值为(889.24±62.00)mGy.cm,对照组为(1185.04±140.05)mGy.cm,两组差异有统计学意义(P<0.05)。两组图像质量主观评分差异无统计学意义(P>0.05),头颈部血管CT值等级评分中大脑中动脉两组差异有统计学意义(P<0.05),其它血管两组差异无统计学意义(P>0.05)。结论:100kVp、低对比剂剂量头颈CTA检查所得图像能够满足诊断需求,同时降低了辐射剂量和对比剂用量。
Objective:To evaluate image quality and radiation dose of multidetector computed tomography (MDCT) for carotid CT angiography (CTA) with low kVp and low dose contrast media. Methods:Carotid CTA was performed in 30 patients (group A) with 100kVp and low dose contrast media (0.3mL/kgbw). Carotid CTA was performed in another 30 patients (group B) with 120kVp and 60mL contrast media. Radiation exposure parameters (SSDE and DLP) were compared with independent samples t tests. Image quality was assessed and CT attenuation was measured. Results: Both the mean values of SSDE and DLP of group A were significantly lower than those of group B(24.29±0.98)mGy vs(31.77±2.27)mGy (t=-16.60,P〈0.01) ;(889.24±62.00)mGy.cm vs (1185.04±140.05)mGy.cm(t=-10.58,P〈0.01). There was no significant difference between the overall image qualities of the two groups. Conclusion:Carotid CTA with 100kVp and low dose contrast media can provide satisfactory images with significant reduction of radiation exposure and iodine administration simultaneously.
