多普勒组织成像对冠心病患者室壁运动速度的研究 被引量:23

A study of regional wall movement velocity of left ventricular with Doppler tissue imaging in coronary artery disease patients

摘要 目的 探讨脉冲波多普勒组织成像 (PW DTI)定量诊断冠心病局部室壁运动异常的临床应用价值。方法 应用PW DTI技术对 5 4例临床确诊 (其中 30例经冠脉造影证实 )的冠心病患者和 30例正常对照者左室壁节段长轴及短轴方向运动速度进行测定 ,并与常规二维超声心动图 (2DE)和冠状动脉造影结果对照分析。结果 冠心病患者 2DE显示运动异常的室壁节段 ,PW DTI所测峰值运动速度除后壁三个水平短轴方向A波与正常组相应节段无明显差异外 ,余各速度均明显减低。左室壁节段短轴与长轴方向S波及E波峰值速度的分布比例在冠脉轻度狭窄组与正常组间无明显差异 ,在中度狭窄组及重度狭窄组与正常组相比均有显著性差异。冠心病运动异常节段短轴方向峰值运动速度 ,92 .4%的室壁节段S≤ 6 .0cm/s ,94.2 %的节段E≤ 7.0cm/s ;长轴方向峰值速度 ,96 .7%的节段S≤ 7.0cm /s ,96 .1%的节段E≤ 8.5cm/s。而正常组相应的百分比分别为4.6 %、6 .3 %和 3 .0 %、1.9%。结论 PW DTI可定量评价冠心病局部室壁长轴与短轴方向运动异常。短轴方向峰值速度S≤ 6 .0cm/s、E≤ 7.0cm /s ,长轴方向峰值速度S≤ 7.0cm /s、E≤ 8.5cm /s 。 Objective To explore the clinical value of Doppler tissue imaging (DTI) in detecting of regional wall movement abnormality (RWMA) in patients with coronary artery disease (CAD).Methods The moving velocities of left ventricular wall and IVS in short axis direction and in long axis direction were measured and analyzed by DTI technology in 54 consecutive patients with CAD clinically proved (30 of them were confirmed by coronary angiography) and 30 normal control subjects.The results of DTI were also compared with conventional echocardiographic findings.Results Color of hypokinetic, akinetic and dyskinetic segments detected by twodimensional echocardiography in CAD patients became dark or disappeared or showed a opposite color in twodimensional or Mmode DTI.In PWDTI mode profile these segments showed significantly lowered velocity amplitude except for a wave of post wall in 3 levels. For velocity distribution of S and E wave in long and short axis, there was no difference between mild stenosis group and normal control group, but there was significant difference between moderate or severe stenosis group and normal control group. For peak velocity in short axis direction of RWMA segments, 92.4% segments S ≤ 6.0 cm/s and 94.2 % segments E ≤ 7.0 cm/s; in long axis direction, 96.7 % segments S ≤ 7.0 cm/s and 96.1 % segments E ≤ 8.5 cm/s. The corresponding percentages of normal group were 4.6 %, 6.3 % and 3.0 %, 1.9 %,respectively. Conclusions PWDTI can assess RWMA quantitatively both in short and long axis direction. It is demonstrated that S ≤ 6.0 cm/s and E ≤ 7.0 cm/s in short axis direction, S ≤ 7.0 cm/s and E ≤ 8.5 cm/s in long axis direction can be used as the criterion of RWMA.
