摘要
目的观察口服普瑞巴林联合医用臭氧椎旁阻滞及疱疹区皮内注射治疗老年患者躯干部带状疱疹后神经痛(PHN)的疗效及不良反应。方法选择病程超过3个月的躯干部PHN老年患者49例,随机分为口服普瑞巴林组(A组,n=26)和口服普瑞巴林联合医用臭氧椎旁阻滞及疱疹区皮内注射组(B组,n=23)。治疗前,治疗后2、4 d,1、2、4、6、8周对患者进行视觉模拟评分(VAS)和睡眠评分,观察不良反应。通过VAS及患者满意度评定疗效,比较两种治疗方式的疗效。结果两组患者治疗后各时间点VAS及睡眠评分较治疗前均明显下降(P<0.01),与A组比较,B组患者治疗后2、4 d,1周VAS降低更明显,治疗后2 d睡眠评分低于A组(P<0.05);B组显效率(69.6%)明显高于A组(38.5%),差异有统计学意义(P<0.05)。结论口服普瑞巴林联合医用臭氧椎旁阻滞及疱疹区皮内注射治疗PHN能缩短起效时间,降低患者疼痛程度,改善患者睡眠质量,治疗效果良好,不良反应发生率低。
Objective To observe the clinical effects of oral pregabalin combined with paravertebral block and intracuta- neous injection using medical ozone in the treatment of elderly trunk postherpetic neuralgia （PHN）. Methods 49 elderly patients with trunk PHN and the course of disease more than 3 months were selected and randomly divided into group A（n=26） and B （n=23）. The group A was treated by oral pregabalin and the group B used the same dose of pregabalin combined with paraverte- bral block and intracutaneous injection with medical ozone. The visual analog scale （VAS） and the sleep quality score were e- valuated before treatment and on 2,4 d and 1,2,4,6,8 weeks after treatment and the adverse reactions were observed. The cu- rative effects were compared between the two groups. Results The VAS scores and the sleep quality scores in each timepoints after treatment in the two groups were obviously decreased than those before treatment（P〈0.01 ）. Compared with the group A, the VAS score and the sleep quality score on 2,4 d and 1 week after treatment in the group B were decreased more obviously, and the sleep quali^y score on 2 d after treatment was lower than that in the group A （P〈0.05）. The significantly effective rate in the group B was 69.6% ,which was obviously higher than that in the group A（38.5% ,P〈0.05 ）. Conclusion Oral pregabalin combined with paravertebral block and intracutaneous injection by medical ozone in treating elderly trunk PHN can shorten the onset time, decrease the pain degree and improve the sleep quality with low incidence rate of adverse reactions.
