摘要
目的探讨管状吻合器在结肠造瘘术中的应用价值.方法回顾性分析2010年1月-2012年7月苏州大学附属第一医院普外科行miles结肠永久性造瘘术的41例直肠癌患者的资料,其中传统手工缝合的23例,应用吻合器造瘘的18例,对两组的造口时间、术中出血量、术后人造肛门第一次排气时间、平均住院时间分析比较,观察是否存在乙状结肠造瘘口感染、缺血坏死、出血、造瘘口回缩,随访观察是否造瘘口狭窄、造瘘口旁疝.结果两组的术中出血量、术后排气时间以及造瘘口的并发症并无明显的统计学差异,但吻合器组在造口时间和平均住院时间方面有明显的优势.结论应用吻合器行miles结肠永久造瘘,具有减少手术时间、操作简单、形态美观便于护理,手术并发症少的优点,值得推广.
Objective To evaluate the clinical value of circular stapler in sigmoid colostomy.Methods Reviewing the data of 41 patients with rectal cancer,including 23 cases of the traditional hand-stitched and 18 cases with application stapler colostomy in The First Affiliated Hospital of Soochow University from January 2010 to July 2012.The medical records including operation time,blood loss,postoperative artificial anus exhaust time,the average hospital stay,whether the existence of the sigmoid colon stoma infection,necrosis,hemorrhage, retraction,follow-up observation stoma stenosis,stoma hernia were compared between the two groups.Results There were no statistically significant difference in blood loss,postoperative discharge time and the complications of the stoma,but in aspects of the operation time and the average hospital stay,the group of stapler had a distinct advantage.Conclusion The circular stapler in sigmoid colostomy is worth being used widely with its advantage in reducing the operation time,simple operation,beautiful shape,easy to care and fewer surgical complications.
