切皮前后应用帕瑞昔布钠对乳腺癌根治术患者术后镇痛和细胞因子的影响

摘要 目的研究切皮前后应用帕瑞昔布钠对乳腺癌根治术患者术后镇痛效果和细胞因子的影响。方法选择ASAⅠ～Ⅱ级择期在全麻下行乳腺癌改良根治术患者60例,随机分为2组:Ⅰ组切皮前30 min行帕瑞昔布钠40 mg静脉注射;Ⅱ组切皮后30 min行帕瑞昔布钠40 mg静脉注射。分别于手术后1 h、6 h、18 h和24 h评估疼痛评分,分别于切皮前、缝皮时和手术后24 h检测血清白细胞介素-6(IL-6)和肿瘤坏死因子(TNF-α)浓度。结果 2组患者的VAS评分无显著性差异。Ⅰ组中舒芬太尼的用量在术后18 h和24 h均明显少于Ⅱ组。Ⅱ组血清IL-6的浓度在手术后24 h较Ⅰ组明显升高。结论切皮前应用帕瑞昔布钠较切皮后应用能够减少手术后舒芬太尼的用量。
