摘要
目的探讨乙型肝炎患者肝移植术后避免乙型肝炎病毒(hepatitis B virus,HBV)再复发的临床治疗要点与护理措施。方法分析2009年3月至2011年12月温州医学院附属第一医院92例乙型肝炎肝硬化及肝癌患者肝移植术后防治HBV再感染的治疗方案,总结防治期间的护理要点。结果所有患者随访3～24个月,其中1例患者在肝移植后5个月肝癌复发死亡;6例患者分别在术后1～18个月HBV复发,复发率为6.6%(6/91);85例患者HBsAg阴性。结论通过制定个体化的防治方案以及护理人员精心的护理、正确的用药指导、及时监测患者不良反应并增减用药剂量,有效降低了乙型肝炎患者肝移植术后HBV的再感染率,提高了患者的生活质量。
Objective To explore the clinical treatment and nursing strategies of HBV recurrence after liver transplantation for patients with HBV. Methods Data of 92 patients with liver cirrhosis or hepatocel- lular carcinoma with HBV after liver transplantation from the First Hospital of Wenzhou Medical College were collected and analyzed to summarize the way of prevention and points of nursing from Mar. 2009 to Feb. 2012. Results All of the patients were followed for 3-24 months, with one patient died of reoccurrence of hepatocellular carcinoma in five months after liver transplantation, six patients relapsed in 1 - 18months after operation, and the recurrence rate is 6.6%, 85 patients with no HBV recurrence. Conclusion Life quality was im- proved and recurrence rate was decreased by making individualized preventive and control plan, meticulous care and medicine guidance, side effect monitoring and timely adjustment of the medicine dose.
